放射学实践 2009年 3月第 24卷第 3期 RadiolPractice，Mar2009，Vol24，No．3 305 骨 骼肌 肉影 像 学 周 围神经鞘瘤的CT和 MRI表现 丁小南，袁建华，王志平 【摘要】 目的：探讨周 围神经鞘瘤 的CT和 MRI表现。方法 ：回顾性分析 13例经病理证实的周 围神经鞘瘤 的 CT和 MRI表现 ，其 中发生在颈椎和胸椎椎 间孔 内的2例、颈部 1例、枕部 1例、纵隔 l例、胸壁 1例及下肢 7例 。结果：肿瘤均 呈椭圆形，边界清楚光滑 ，沿神经走行方向生长，2例有邻近椎 间孔扩大。cT显示肿瘤包膜密度高，包膜 内肿瘤组织密度 低于肌 肉组织。MRITwI显示肿瘤呈 中～低信号 ，TwI呈不均匀高信号 ，增强后有不均匀强化。其 中显示靶征 4例 ， 神经出入征 3例 ，脂肪包绕征 8例 ，脂肪尾征 7例及脑脊液尾征 1例 。结论 ：周 围神经鞘瘤 的影像学表现有一定特征性 ， CT和 MRI对诊 断有重要价值 。 【关键词】 神经鞘瘤 ；体层摄影术 ，X线计算机 ；磁共振成像 【中图分类号】R445．2；R814．42；R745 【文献标识码】A 【文章编号2009)03—0305—04 CT andMRIFeaturesofPeripheralNeurilemoma DING Xiaonan，YUAN Jian—hua，WANG Zhiping．Departmentof Radiology，PeopleSHospitalofZhejiangProvince，Hangzhou310014，P．R．China [Abstract] Objective：ToanalyzetheCTandMRIfeaturesofperipheralneurilemoma．Methods：TheCTandMRIlea turesof13patientswithhistopath0logyprovedperipheralneurilemomawereretrospective1yanalyzed．Thelocationofthe tumorwascervico—thoracicintervertebralforamen (2cases)，neck，occipitalregion，mediastinum ，chestwallfor1caseeach， andlowerlimb (7cases)．Results：Allofthetumorswereovalshaped，well—definedandwiththeextentalongthenervedis— tribution．Enlargementofintervertebralforamencouldbeassessedin2cases．Thecapsuleoftumorsshowedhyper—attenua— ted。thedensityofintra—capsulartumormasswaslowerthanthatofmuscle．Themassesshowediso一／hyposignalintensities onMR TlW Iandheterogeneoushyper—intensitiesonT2W I．Aftercontrastadministration，thelesionsshowedinhomoge— neousenhancement，with “target”signsin4cases，nerveenteringandexitingthetumorin3cases，fat—encirclingsign in8 cases。fat—tailsign in7cases，cerebrospinal—fluidtailsign in 1case．Conclusion：Characteristicimagingfindingscouldbere— vealedinperipheralneurilemoma，whichmightprovidesignificantinformationforthediagnosis． [Keywords] Neurilemoma；To
