小针刀神经根管直接松解术治疗腰突症随机双盲对照临床研究 田和炳 李兴勇 蒋晓明 吴 蓉 郭 旗 谢 弢 四川省德阳市旌阳区中医院（人民医院）康复科，四川德阳618000 ［摘要］ 目的 在前期研究基础上，进一步研究小针刀神经根管直接松解术治疗腰突症临床疗效及安全性。方法 50例腰突症患者按随机双盲对照法分为治疗组25例，给予针灸、推拿、牵引及中药辨证施治内服小针刀神经根管直接松解术对照组神经根管假松解术。结果 治疗组效率明显于对照组（P0.05)，两组总疗程无显著差异（P0.05）治疗组小针刀操作中均发生下肢串麻感，治疗及随访中未见神经损伤发生结论 小针刀神经根管直接松解术治疗腰突症具有显效快、疗效好操作简便、痛苦小、潜在风险低的优势。 ［关键词］小针刀；神经根管直接松解术；神经根管松解术；腰椎间盘突出症 Clinical Study of Direct Neurolysis on Radicular Canal with cupotomy in Treating Lumbar Disc Protrusion Hebing Tian, Xingyong Li▲, Xiaoming Jiang, Rong Wu, Qi Guo, Tao Xie Rehabilitation Department, The Traditional Chinese Medicine Hospital of Jing yang District of De yang, Si chuan De yang. 618000 ［Abstract］Objective To esearch further in the effect and security of the direct neurolysis on radicular canal with acupotomy(DNRCA) to treat the lumbar disc protrusion on the basis of the preliminary study. Methods 50 cases of the lumbar disc protrusion were randomly divided into two groups: the treatment group and the control group. On the basis of both groups treating with the acupuncture, massage, traction and the Traditional Chinese drugs based on differentiation, the patients of the treatment group were treated with DNRCA and the control group with the pseudo acupotomy. The study was end when the patients felt no effect or aggravated. All patients was evaluated with the cumulative score of the symptoms and signs（SS）, MPQ and the treatment and visiting period. Return visit was lasted half a year after treatment. Results The effect of treatment group was obviously superior to that of the control group(P0.05). The period of treatment had no significant difference between the two groups (P0.05). Meanwhile, all of the patients in the treatment group occurred the electric feeling witch following the never distribution during the acupotomy therapy, but the potential risk of nerve damages was not detected.
