微创全髋关节置换治疗老年股骨颈骨折 本文档由【中文word文档库】提供，转载分发敬请保留本信息； 中文word文档库免费提供海量范文、教育、学习、政策、报告和经济类word文档。 周明旺，李盛华，柳海平，王承祥 (甘肃省中医院， 甘肃 兰州 730050) 【摘要】目的：探讨分析微创全髋关节置换术(THA)治疗老年股骨颈骨折的安全性、可行性及临床效果。方法：观察63例微创THA治疗老年股骨颈骨折的手术时间、切口大小、出血量及术后髋关节功能等指标,并与56例常规THA治疗老年股骨颈骨折相比较。结果：微创THA组与常规THA组在术前诊断、年龄、体重、术前髋关节Harris评分、合并症等方面无显著差异，两组在失血量(手术失血量及术后引流量)、输血量、切口长度、术后疼痛等方面均存在显著差异(P< 0.05)，术后两组髋关节功能Harris评分无统计学差异(P>0.05)。结论：微创THA技术治疗老年股骨颈骨折具有创伤小、出血少、恢复快等优势，可获得与常规THA相同的治疗效果，适合在临床工作中推广应用。 【关键词】股骨颈骨折；全髋关节置换术；微创性；老年人 Minimally Invasive Total Hip Replacement for Treating Femoral Neck Fracture in the Elderly ZHOU Ming-wang, LI shen-ghuan, LIU Hai-ping, WANG Cheng-xiang, Department of Orthopaedics ,the TCM Hospital of Gansu, Lanzhou 730050, Gansu , China Abstract Objective：To discuss the security, feasibility and clinical effects of minimally invasive total hip arthroplasty for the treatment of femoral neck fractures in the elderly．Methods：63patients with femoral neck fractures in the elderly were accepted minimally invasive approach hip arthroplasty .Compared operation time,incision size, amount of bleeding, postoperative hip function and other indicators with the traditional hip replacement. Results: There was no remarkable difference between the two groups in the aspect of pre-operation diagnosis，age，weight，Harris score(P>0.05)．But the comparison between the two groups showed that the minimally invasive surgical technique resulted in less blood loss，less pain，quicker return to function and fewer post-operation complications(P< 0.05)．Conclusion：minimally invasive total hip arthroplasty has the advantages of less soft tissue damage，less blood loss and faster recovery，which can improve hip joint function as good as that of traditional posterior approach，especially good for the elderly patients with femoral neck fractures． Key words: Femoral neck fracture；Tota1 hip arthroplasty；minimal invasive； Aged 目前，微创技术的发展把人工髋关节手术提高到一个新的水平。在常规全髋关节置换(total hip arthroplasty，THA以最小的侵袭和最小的生理干扰达到最佳外科疗效THA治疗老年移位性股骨颈
