厦门大学新生奖学金年度评审 Annual Review for XMU Scholarship Students  OSAO 2010-4-22 年度评审 Annual Review 年度评审情况简介 Introduction 年度评审流程 Procedure 表格指导 Direction of form filling-in 保险介绍 Introduction 凡来华学习时间超过六个月的留学生必须在注册时购买中国平安保险公司提供的外国留学生团体综合保险; Overseas students to study in China for over six months are supposed to join a group insurance scheme. 个人人身保险的保险单不能作为办理入学注册和在华居留许可的文件.保险手续在入学时办理; No student will be registered without proof of adequate insurance. Students can purchase their insurances in September each year. 团体综合险保险费为600元人民币/人/年;到期后需按要求续保 An group insurance scheme charges 600 RMB/person/year. A renewal is required in every September organized by XMU. 保险内容 Contents 平安险(身故或残疾定额赔付） Life insurance against death and dismemberment 人身意外伤害医疗险 Accidental Medical Treatment Insurance 住院医疗保险 Hospitalization Medical Insurance 注意：门诊治疗不可报销 Refunds on out-patient treatments are excluded 谢谢！THANK U! * * 主要内容 Contents 年度评审 Annual Review 保险 Insurance 对象：已在校奖学金生，学习期限超过一年或毕业前申请延长者 Candidate：Current XMU students whose scheduled study duration exceeds 1 year and those applying for CSC scholarship extension before graduation 评审时间：每年五月第2-3个星期 Time: the 2nd and 3rd weeks of May 评审单位：各学院及国际处 Evaluator: supervisor/ college and International Office (OSAO) 评审要求：学习成绩、学习态度和考勤情况、行为表现和奖惩 Qualification: course results, attendance and conduct 评审结果：决定是否能继续享受奖学金资格 Eligible CSC students will keep the scholarship status next year. 年审简介 Introduction 领取表格并填写 Step 1: Get the form and complete the required statement 学院确认并审核盖章 Step 2: Get the academic report of last semester + Send to the supervisor/department for a primary review 表格递交至国际处 Step 3: Submit the form and attachments to OSAO *递交截止日：2010年5月14日 Deadline for form collection in OSAO: May 14th , 2010 年审流程 Procedure ?详细填写过去及目前个人学习、表现情况 Your personal statement has to be included: 1 review on academic
