雄激素受体基多因态性与男性冠心病关系的研究.docVIP

【摘要】 　　目的：雄激素受体（A R ）基因外显子1 区基因片断C A G 多态性与男性冠心病（C A D ） 及其危险因素的关系。方法：125 例冠脉造影证实的男性冠心病人均接受检查，C A D 严重程度采用冠脉狭窄≥ 50 %的支数（0 ， 3 ），分别计算心肌梗塞栓塞（TIMI）危险积分，外周血A R 转录激活区C A G 重复序列的长度采用PC R 法测定。结果：C A G 重复长度为13 ， 30 次，C A G 重复≥ 24 次者为长A R 基因组，＜ 24 次者为短A R 基因组。与长A R 基因组比较：短A R 基因组患者具有较低的平均体重指数（B MI）（t=-31024 ，P =01005 ）和较低水平的高密度脂蛋白（H D L ）（t=-21610 ，P =01010 ）；短A R 基因组出现明显冠脉狭窄的可能性较大（x2 =4182 ， P =01028 ），TIMI危险积分较高（M -W 检验，z=-21644 ， P =01008 ）。Spearman 相关分析表明，A R 基因C A G 长度和TIMI 危险积分显著负相关（r=-01230 ，P =01010 ），这种相关性独立于年龄、B MI 、L D L 、U A 和H D L 之外（偏相关系数r=-01250 ，P =01006 ）。结论：具有短A R 基因者与更加严重的冠心病相关，提示C A G 多态性与男性C A D 病情发展相关，A R 表达增多，雄激素敏感性增高可能是男性C A D 发病率较高的原因之一。 【关键词】 受体，雄激素；基因；冠状动脉疾病 　　 Correlation between the androgen receptor gene polymorphism and the severity of coronary artery disease 　　Z H A N G Xiao hong ，R O N G Zhi　yi 　　Chinese Journal of Cardiovascular Rehabilitation Medicine ，2007 ，16 （4 ）：331 Abstract ：Objective ：To investigate the correlation between the androgen receptor gene C A G polymorphism in the exon-1 do main and the severity of coronary artery disease（C A D ）.Methods：The nu m ber of C A G repeatin the androgen receptor （A R ）gene transactivation domain in 125 male patients（36 ， 86 years old ）proven C A D by angiography wasexamined .The severity of C A D was assessed by the nu m ber（0 ， 3 ）of coronary vessels with ＞ 50 %reduction in lu2minal diameter ，the thro m bosis in myocardialinfarction （TIMI）risk score of each was calculated .Results ：The lon2ger A R was defined as C A G repeat nu m ber ≥ 24 times ，accordingly ，the shorter was ＜ 24 times .The mean body massindex（B MI）was significantly lower（t=-31024 ，P =01005 ），the H D L level was also significantly lower （t=-21610 ，P =01010 ）in patients with short A R than those in men with long A R .Patients with short A R had C A D morefrequently than that of men with long A R （x2 =4182 ，P =01028 ）.The TIMI risk score oflong A R was significantlylower than that of shorter（M W test ，z=-21644 ，P
