严重胸部创伤合并肺挫伤的早期诊断与治疗（Early diagnosis and treatment of severe chest trauma combined with pulmonary contusion） Early diagnosis and treatment of severe chest trauma combined with pulmonary contusion 2009-10-18, Xia Xiaoming, Zhou Ji, Zhao Hanwei, Li Zhaohui [Abstract] Objective To investigate the early diagnosis and treatment of severe pulmonary contusion. Methods 68 cases of severe pulmonary contusion (injury severity score ISS = 20 points) to analyse and summarize the results of early diagnosis and treatment experience of the patients in 68 cases, 52 cases were cured (76%), 16 cases of death (24%). There were 31 cases of ARDS, 40 cases of pneumonia and 8 cases of atelectasis in the course of treatment. Conclusion hemoptysis or bloody sputum, lung sound anergy and dyspnea are the major manifestations of severe pulmonary contusion. Chest X ray and CT examination are reliable evidence for diagnosis. PaO2/FiO2 is an important index to determine the severity of hypoxemia. Respiratory tract patency, anti shock, active treatment of combined injuries, rational use of mechanical ventilation, hormones and antibiotics are important methods for the treatment of severe pulmonary contusion. (Department, of, Thoracic, Surgery, Xiaogan, Municipal, Central, Hospital, Hubei, 432100) Abstract Objective To explore the early diagnosis and treatm ent of severe pulmonary contusion.Methods Sixty-eight cases with severe pulmonary contusion with ISS were selected and the = 20, early diagnosis and treatment were discussed.Results Fifty-two cases survived and cases died.During hospital, 16 ization, there were 31cases complicated with ARDS cases with pneumonia, 40 and 8 c ases with atelectasis.Conclusion Hemoptysis or sputum with blood, rales over the lungs and dyspnea are the major manifestations of severe pulmonary contusion.C
