663例胶囊内镜检查回顾性研究分析 干 甜，杨小军，王 雷，饶春燕，张志梅，樊超强，赵晓晏 (第三军医大学新桥医院消化内科,重庆 400037) [摘要] 目的 探讨胶囊内镜对消化道疾病的临床诊断价值。方法 回顾性分析663例次（其中健康体检者176例）采用国产OMOM胶囊内镜行消化道检查的临床资料。结果 663例受检者中，658例顺利完成检查，成功率99.2%，528例发现阳性病变，检出率为79.6%；共检出病变1121处（403例同时存在2种或2种以上病变）。487例患者中，小肠病变检出率为61.8%，诊断率为55.0%。所检出的病变包括糜烂、溃疡、息肉、血管发育异常、憩室、寄生虫、淋巴管扩张、肿瘤等。胶囊在食道内运行时间28.2±11.8（3～58 ）s，在胃内运行时间57.4±48.4（2-204）min；在小肠内运行时间268.7±96.9（54-525）min。4例患者发生胶囊内镜滞留。结论 胶囊内镜检查安全、有效，耐受性好，诊断率较高，对消化道疾病特别是小肠疾病具有重要的诊断价值。 [关键词] 胶囊内镜；小肠疾病；诊断价值 [中图法分类号] [文献标志码] A Retrospective analysis of capsule endoscope in 663 patients Tian Gan Xiao-jun Yang Lei WANG Chun-yan Rao Zhi-mei Zhang Chao-qiang Fan Xiao-yan Zhao(Department of Gastroenterology, Xinqiao Hospital, Third Military Medical University, Chongqing, 400037) [Abstract] Objective To explore the diagnostic value of capsule endoscope in patients with digestive tract disorders. Methods A retrospective analysis of clinical data was made on capsule endoscopy examination for 663 cases（including 176 health check volunteers） . Results Among the 663 cases, 658 cases(99.2%) successfully completed the procedure. Abnormal findings were observed in 528 cases(79.6%). The significant pathological lesions were revealed in 1121 out of 528 patients. 403 cases of the patients had two or more than two lesions simultaneously. The small intestinal diseases were observed in 301 patients among 487patients (61.8%). The diagnostic yield was (55.0%). Among the various findings,the capsule detected erosions, ulcers, polypus, angiodysplasia, diverticulum, parasite, lymph duct expansion and tumor. The average running time of the capsules in the esophagus ranged from 3 to 58 seconds（28.2±11.8s）. The running time of the capsules in the stomach ranged from 2 to 204 minutes(57.4±48.4 min). The running time of the capsules in the small intestine ranged from 54 to 525 minutes(268.7±96.9min). Capsule retention was happened in 4 caces. Conclusions The capsule endos
