膝骨关节炎患者关节液中IL17表达水平临床探究 　 作者：董军峰, 郑之和, 李新志, 卢国强, 曾勇 【摘要】 目的 探讨膝骨关节炎(knee osteoarthritis,KOA)患者膝关节液中IL17表达水平。方法 用双抗体夹心ELISA法测定37例(41膝)KOA患者与24例(24膝)志愿者膝关节液中IL17浓度并比较两组差异。将KOA患者按综合评分法分为轻(13膝)、中(19膝)、重(9膝)三组，比较三组患者膝关节液中IL17浓度的差异。结果 KOA患者膝关节液中IL17浓度为156.76±112.78 pg/ml，明显高于对照组31.67±18.74 pg/ml（Plt;0.01）。轻、中、重三组患者膝关节液中IL17的浓度分别为50.16±24.15 pg/ml、154.63±47.39 pg/ml、314.83±105.89 pg/ml，呈现出随着病情分级逐渐加重而IL17表达水平逐渐增高的趋势(Plt;0.01)。结论 IL17作为一种前炎症细胞因子，参与了KOA疾病的诱发、触发了病理改变、加快了病情的进展。IL17可作为判断KOA临床病情、预后的免疫学指标之一。对IL17的研究将有助于进一步在分子水平阐明KOA的发病机制，为临床诊断和治疗KOA提供新的思路。 【关键词】 膝骨关节炎； IL17； 表达； ELISA 【Abstract】 Objective To explore the expression of the IL17 in the knee osteoarthritis(KOA) patients sick knee joint synovia. Methods The concencentrations of the IL17 in the knee joint synovia were detected by two antibodies sandwichELISA method and compared between the 37 cases(41 knees) of the KOA patients and 24 cases(24 knees) of volunteers. The KOA patients were divided into the mild group(13 knees), the moderate group(19 knees) and the severe group(9 knees).The concentrations of the IL17 in the knee joint synovia were compared in the three groups. Results The concentration of the IL17 in the sick knee joint synovia of the sick group was 156.76±112.78 pg/ml, which was significantly higher than the contrast group (31.67± 18.74 pg/ml) (Plt;0.01). The concentration of the sick knee joint synovia of the mild group, moderate group and severe group were 50.16 ±24.15 pg/ml, 154.63±47.39 pg/ml and 314.83± 105.89 pg/ml, respectively, which appeared agradual elevatory uptrend in the state of illness? development(Plt;0.01). Conclusions IL17 participates the KOA’s evocation, trigges the alteration in pathology, speeds up the progression of the pathogenetic condition, as a preinflammation factor. IL17 can be an immunological index of judging the clinical pathogenetic conditions and prognosis for the KOA patients to offer a new idea to the diagnosis and therapy of KOA. 【Key words】 knee o
