Containment 围堵措施 5C / FA-CA Failure Analysis 失效分析 Simple analysis - Describe observation on the failure sample with naked eye, micro scope, function test Detail analysis - Physical, Electrical, Chemical analysis result Conclusion of Root Cause Analysis 根本原因分析结论 Describe actual root cause – based on 5M (Material, Method, Man, Machine, Mother nature) Conclusion must be logical and convincing enough. Cause 原因 5C / FA-CA Corrective Action: 纠正措施 The action should be able to remove the source of the problem by improving 5M. There could be short term and long term. - Short term could be effective screening method for future shipment. - Long term could be design change, and eliminate the real source of problem. Preventive Action: 预防措施 The action should be systematic so that the same kind of problem won’t happen again on the other products as well. Lessons learned database - Improve design process as an example. Corrective Action 纠正预防措施 5C / FA-CA Verification 确认 Check the effectiveness with on-site engineer if we conduct pre-sorting.
