中晚期妊娠引产出现忽略性横位应用杜非合剂联合缩宫素临床研究 　　【摘要】 目的 中、晚期妊娠引产出现忽略性横位后应用杜非合剂联合缩宫素静脉点滴减少剖宫产取胎及产后出血的临床研究。方法 ①晚期妊娠引产过程中出现忽略性横位53例,分为对照组(26例)治疗组(27例),治疗组应用杜非合剂联合缩宫素静脉滴注治疗,对照组静脉推注地西泮联合缩宫素治疗;②观察剖宫取胎率、产后出血率、自然分娩率、总产程时间、毁胎术率情况。结果 ①治疗组与对照组排胎时间比较:治疗组较对照组用药至规律宫缩时间(潜伏期)明显缩短,差异有统计学意义(P0.05)。治疗组较对照组用药后至胎儿胎盘娩出时间缩短,差异有统计学意义〔P0.05)。结论 ①杜非合剂配合缩宫素有效镇静、镇痛、松弛宫颈并协调有效宫缩缩短总产程时间从而达到了降低产后出血及剖宫取胎的发生;②此方法经济实惠、应用方便,适用于广大基层医院。 　　【关键词】 杜非合剂;忽略性横位;缩宫素;中晚期妊娠 　　 　　A study on Dolantin and promethazine in half dose combined with oxytocin in treatment of the artificial labor in middle or late appear the neglected shoulder presentation 　　 　　REN Wei-ru,DUAN Jing.The third hospital Zaozhuang City,Shandong277100, China 　　 　　【Abstract】 Objective The artificial labor in middle or late appear the Neglected shoulder presentation application dolantin and the promethazine in half of the dose association oxytocin Treatment decrease postpartum hemorrhage and uterine-incision delivery of clinical research. Methods ①Fifty-three patients in the artificial labor in late pregnancy arising in the neglected shoulder presentation were divided into two group, with one group(n=27) treated with the half dose of dolantin and promethazine combined oxytocin and the other group(n=26) with diazepam combined oxytocin.②To observe caesarean birth rate, natural birth rate, destructive operation rate, the rate of postpartum hemorrhage. Methods ①The children waiting time of the experimental group and the control group: After treatment, the decrement of the law of contraction time (latency) was more significant in the experimental group than that in the control group(P0.05). The decrement of time from treatment to fetal placental delivery was significant in the experimental group than that in the control group(P0.05).Conclusion ①The half dose dolantin and promethazine combined with intravenous oxytocin United contraction of the uterus can be to shorten the production time, reduce the cesarean section and postpartum hemorrhage;②Economical app
