乳腺良恶性病灶常规超声和超声造影的对比研究 　　[摘要] 目的 分析乳腺良恶性局灶性病灶实时超声造影的声像特征，并与常规超声及彩色多普勒血流比较，探讨其在乳腺病灶的诊断及鉴别诊断的价值。方法 对43例乳腺病例（45个乳腺病灶）进行常规超声及超声造影检查。结果 超声造影诊断乳腺良恶性肿块的灵敏度、特异度、阳性预测值、阴性预测值及准确性均较常规超声检查高。乳腺肿块恶性组的动脉血流速度、阻力指数均高于良性组（P0.05）。结论 实时超声造影的诊断价值高于常规超声检查，提高对乳腺良恶性病灶诊断效能，有助于乳腺良恶性鉴别诊断。 　　[关键词] 乳腺病灶；超声造影；灌注模式 　　[中图分类号] R445.1 [文献标识码] A [文章编号] 2095-0616（2017）11-164-03 　　Comparative study on conventional ultrasound and contrast-enhanced ultrasound of benign and malignant breast lesions 　　XIE Junming1 TANG Qing2 HE Liantu2 ZHOU Dazhi2 WU Bijun2 　　1.B-Ultrasound Room， Stomatological Hospital Affiliated to Guangzhou Medical University， Guangzhou 510140， China； 2.Department of Ultrasound， First Hospital Affiliated to Guangzhou Medical University， Guangzhou 510120， China 　　[Abstract] Objective To analyze imaging characteristics of real-time contrast-enhanced ultrasound of benign and malignant breast lesions， to compare with blood flow of conventional ultrasound and color doppler and to explore its value in diagnosis and differential diagnosis of breast lesions. Methods 43 breast cases（45 breast lesions） were given conventional ultrasound and contrast-enhanced ultrasound. Results Sensitivity， specificity， positive predictive value， negative predictive value and accuracy score of contrast-enhanced ultrasound in diagnosis of benign and malignant breast tumor were all higher than those of conventional ultrasound examination. Artery blood flow velocity and resistance index of the malignant breast tumor were both higher than those of the benign breast tumor group. The difference was statistically significant（P0.05）. Conclusion Diagnostic value of real-time contrast-enhanced ultrasound was higher than that of conventional ultrasound examination. It can improve diagnostic efficacy of benign and malignant breast lesions and it is helpful for differential diagnosis of benign and malignant breast. 　　[Key words] Breast lesions； Contrast-enhanced ultrasound； Perfusion pattern 　　乳腺癌是女性常?的恶性肿瘤之一。近年来，统计研
