非医嘱离院方式构成比原因剖析及改进措施 　　[摘要] 目的 通过对非医嘱离院方式构成比统计及深入分析，从中发现医院质量管理中的问题并找出改进措施，使医院医疗质量和服务水平得以提高，加强医院管理。 方法 调取2013年1～12月离院方式数据，计算出上、下半年的非医嘱离院方式构成比，分析上半年高于下半年的原因，对实施改进措施前后半年数据进行统计分析。 结果 实施改进措施前非医嘱离院方式患者高达3006例次，占全院出院患者12.41%，实施后非医嘱离院方式患者为2173例次，占比例为7.57%，差异有统计学意义（P0.05）。 结论 统计分析非医嘱离院方式构成比指标，对存在问题进行改进，加强了医院管理的同时也提高了经济效益和社会效益，这一指标应引起重视。 　　[关键词] 非医嘱离院；改进措施；医院管理 　　[中图分类号] R197.323 [文献标识码] B [文章编号] 2095-0616（2016）01-180-04 　　[Abstract] Objective To collect and analyze deeply the constituent ratio of non-doctors advice of departure from the hospital， and find that hospital quality management issues and identify ways to improve measures to improve medical quality and service levels， and strengthen hospital management. Methods The measures date of departure from the hospital from January 2013 to December 2013 was collected， calculated constituent ratio of non-doctors advice leaving hospital in the first half year and the second half， analyzed the reason why the first half year was higher than the second half， six months before and after the implementation of the improvement measures data was analyzed. Results Before measures improved， the numbers of patients with non-medical advice away from the hospital were up to 3006， accounting for 12.41% of patients discharged in hospital， non-prescription away from hospital patients were 2173 after measures improved， the proportion was 7.57 percent， and the chi-square test had significant differences. Conclusion From the statistical analysis of non-hospital way constitute medical advice ratio indicators， there is a problem to improve and strengthen the hospital management but also improve the economic and social benefits， this indicator should pay attention to. 　　[Key words] Non-prescription leave the hospital； Improvement measures； Hospital management 　　非医嘱离院方式构成比是重要的医疗统计指标之一，能客观地衡量医疗质量和服务水平。非医嘱离院指患者未按照医嘱要求而自动离院。如：患者疾病需要继续住院治疗，但患者或家属出于个人原因要求出院，此种出院并非由医务人员根据患者病情决定。本研究对我院2013年上半年非医嘱离院方式构成比这一指标统计和原因分析，结合本院实际情况，采取有效改进措施，进一步提高了我院医疗质量和服务水平，
