米索前列醇预防剖宫产术后出血的临床研究 【关键词】 米索前列醇  摘要：目的：观察米索前列醇用于剖宫产预防产后出血的效果。 方法 ：选择225例剖宫产者，随机分为米索前列醇组、米索前列醇+催产素组及催产素组。米索前列醇组75例，术中胎头娩出时将米索前列醇200μg嚼碎后舌下含服，术后经直肠置入米索前列醇200μg；米索前列醇+催产素组80例，术中胎头娩出时将米索前列醇200μg嚼碎后舌下含服，胎儿娩出后宫体肌内注射催产素20 IU，术后经直肠置入米索前列醇200μg；催产素组70例，胎儿娩出后宫体肌内注射催产素20 IU，再静脉滴注催产素20IU。以上各组观察术中及术后2h内出血量。结果:术中及术后2h平均出血量，米索前列醇组为160± ml；米索前列醇+催产素组为158±；催产素组为276±。米索前列醇组与催产素组比较，差异有极显著性(P＜)。米索前列醇组与米索前列醇+催产素组比较，差异无显著性(P＞)。 结论:米索前列醇促进子宫收缩作用强于催产素，能较好地预防剖宫产术后出血，且用药方便、安全。  关键词：米索前列醇；剖宫产术；产后出血  Clinical Study on Prevention of Post Cesarean Section Bleeding with Misoprostol  CHEN Xiu-hua  ( The Second Peoples Hospital of Shantou, Guangdong Shantou15011, China)  Abstract:Objective:　To observe the effects of misoprostol on reduction of postpartum bleeding in cesarcan section. Method:Two hundred and twenty-five cesarean sections were randomized into three groups: misoprostol group (n=75),00μg misoprostol chewed was given sublingually when delivery of baby and00μg misoprostol was given by anus after operation; misoprostol and oxytocin group (n=80),00μg misoprostol chewed was given sublingually and0 IU oxytocin was injected into uterine muscle immediately after delivery of baby and00μg misoprostol was given by anus after operation; Oxytocin group (n=70),0 IU oxytocin was injected into uterine muscle and0 IU intravenous injection immediately after delivery of baby. The amount of bleeding within hours after delivery was measured. Result:The mean amount of bleeding in misoprostol group was 160± ml, in misoprostol and oxytocin group was 158±, oxytocin group was76±, respectively. The difference was significant( P＜) between misoprostol group and oxytocin group, there was no differences between misoprostol group and misoprostol plus oxytocin group(P＞). Conclusion:　Misoprostol is more effective in reduction of postpartum bleeding than that of oxytocin. to prevent post cesarean section administrtion of the drug is easy to perform and safety.
