MRimagingcanbeausefultoolintheevaluationofthehandandwristoftheinjured
athlete.Often,however,providinghelpfulanddiagnosticimagescanbechallengingduetotheneed
forhighspatialresolutionandtheuniquedifficultiespresentedbyimagingthisanatomicregion.In
contrasttoimagingtheknee,forexample,routinepositions,protocolsandimagingnes
frequentlydonotsuffice.Appropriate,clinicallyhelpful,imagingrequiresanaccurate
understandingoftheclinicalhistory,theuseofimagingprotocolsoptimizedforthespecificclinical
scenariounderconsiderationandtheuseofappropriatescanners,coilsandsequences.
Hardware:
Duetothesmallsizeanddifficultyvisualizingimportantstructuresofthehandandwrist,
highspatialresolutionisrequired.Asaresult,theuseofhighfieldscannersisadvantageous.
Unfortunately,however,thebenefitsofincreasedsignaltonoiseassociatedwithincreasedfield
strengthmaybecompromisedbythemagnifiedsusceptibilityartifactwhenscanningpost-
operativepatients.Positioningofthehandandwristmaybeconsiderablyeasierinascannerwitha
widebore.Greaterpatientcomfortwhenusingsmalldedicatedextremityscannerscanalsohelpto
providesuperiorimagesandfewerscansdegradedbymotionartifact.Regardsofthescanner,
optimizedimagingrequiresefficientcoils.Adedicatedwristcoilisgenerallynecessary,though
smallsurfacecoilscanbealsobeusedtoprovidehighresolutionsmallfieldofviewimagesof
smallerstructuresliketheinterphalangealjointsorthenailbed.
Sequences:
Aswithallmusculoskeletalimaging,T1weightedandfluidsensitiveimagesarerequired.
Threedimensionalimagesmaybeacquiredusinggradientechoorfastspinechotechniques.Given
