Thiscasereportdescribesasurgicalorthodonticcasethatusedtherecentlyintroducedsurgery-firstapproachto
correctasevereskeletalClassIIImalocclusion.A19-year-oldwomanpresentedwithseveremandibularprog-
nathismandfacialasymmetry;shehadbeenwaitingforgrowthcompletioninordertopursuesurgicalcorrection.After
predictionofthepostsurgicaltoothmovementandsurgicalsimulation,2-jawsurgerythatincludedmaxillary
advancementanddifferentialmandibularsetbackwasperformedusingasurgery-firstapproach.Immediate
facialimprovementwasachievedandpostsurgicalorthodontictreatmentwasefficientlycarriedout.Thetotal
treatmenttimewas16months.Thepatientsfacialappearanceimprovedsignificantlyandastablesurgicalor-
