根除幽门螺旋杆菌治疗10年消化性溃疡和胃癌的胃镜检出率变化 　　作者：张小艳,王淑英　　作者单位：西安交通大学医学院第一附属医院消化内科，陕西西安 710061 　　摘要：目的 了解根除幽门螺旋杆菌(Hp)治疗10年来胃镜下胃溃疡、十二指肠溃疡、胃癌检出率的变化，探讨疾病的变化趋势及根除Hp治疗的意义。方法 回顾本院1994年1月-1995年12月、2004年1月-2005年12月所有胃镜检查的病例，对比研究胃溃疡、十二指肠溃疡、胃癌的内镜检出率。结果 2004年1月-2005年12月胃溃疡、十二指肠溃疡、胃癌检出率分别为5.53%、10.07%、4.03%，与1994年1月-1995年12月的8.17%、11.21%、6.21%比较显著下降(P0.05);2004年1月-2005年12月胃溃疡、十二指肠溃疡、胃癌的平均发病年龄为(53.34±14.91)岁、(46.94±15.60)岁、(62.05±11.08)岁，分别高于1994年1月-1995年12月的(47.55±13.40)岁、(44.10±13.35)岁、(60.28±11.70)岁(P0.05)。结论 胃溃疡、十二指肠溃疡、胃癌的内镜检出率明显减少且平均发病年龄增大，可能与广泛的根除Hp治疗相关。 　　关键词：胃溃疡,十二指肠溃疡,胃癌;幽门螺旋杆菌;根除治疗 　　ABSTRACT: Objective To investigate the changes in the detection rate of gastric ulcer, duodenal ulcer and gastric cancer by gastroscopy after 10 years eradicative treatment of Helicobacter pylori, in order to learn the characteristic alternations of these diseases and the significance of eradicative treatment of Helicobacter pylori. Methods The clinical and endoscopic data of gastric ulcer, duodenal ulcer and gastric cancer at our hospital in January 1994 to December 1995 and January 2004 to December 2005 were collected and analyzed. Results The detection rate of gastric ulcer, duodenal ulcer, and gastric cancer by gastroscopy during the period of January 2004 to December 2005 (5.53%, 10.07% and 4.03%, respectively) was significantly lower than that during the period of January 1994 to December 1995 (8.17%, 11.21% and 6.21%, respectively, P0.05). The average age of patients with gastric ulcer, duodenal ulcer and gastric cancer between January 2004 and December 2005 was 53.34±14.91, 46.94±15.60, and 62.05±11.08, respectively, which was remarkably higher than that between January 1994 and December 1995 (47.55±13.40, 44.10±13.35, and 60.28±11.70, respectively, P0.05). Conclusion The detection rate of gastric ulcer, duodenal ulcer and gastric cancer by gastroscopy decreased significantly, and the average age of patients increased remarkably.
