Interpreter, Indian Home School Coord, Trade Specialist after 7/1/2004) No tag 02 2 year License: OOS deficiency license Term. 6/30/04 No tag 03 3 year License Term. 6/30/83 No tag 04 3 year License: non-renewable Term. 6/30/04 No tag 05 5 year License (use only for PT, OT, OM, audiology, OTA, PTA, Ed. Interpreter, special ed aide, Indian Home School Coord, Trade Specialist. after 7/1/2004) No tag 07 5 year Non-renewable License: (for ed interpreter only if the begin date is of 7/1/04 or later) The holder must complete the continuing education requirement and all stipulations noted by DPI in the enclosed letter to be eligible for a regular license. 08 Life License This license becomes invalid should the holder remain out of the profession for five or more years. The holder may revalidate this license by completing six semester credits of refresher work or a Professional Development Plan. 09 Emergency Permit The district may request renewal of this permit if there continues to be a shortage of licensed candidates and if the holder completes at least six semester credits or the equivalent in an approved program leading to a license in this area by August 31 of the year this permit expires. 10 Emergency License The district may request renewal of this license if the holder has completed six semester credits or the equivalent in an approved program leading to a license in this area by August 31 of the year this license expires. 11 1 year Administrator License-Board Requested No tag 12 2 year Permit No tag 13 Short-term Substitute Permit (used for those with a non-teaching bachelor’s degree) The district may request renewal of this permit if there continues to be a declared emergency need for substitute teachers in that district.
