Investigation Report Guidelines
ACCIDENT REPORT FORMAT
The aim of the incident/accident investigation is to determine causes of the acciden NOT to apportion blame. Establishing causes helps to identify hazards and to indicate methods of control.
1 SYNOPSIS
Time, date, location, injured person(s)
Accident event
Injury sustained
Summary of findings
2 CONTRIBUTING FACTORS
Material/equipment factors
Environmental factors
People factors
3 RECOMMENDATIONS
Interim corrective measures
Action taken to make the site safe and without risks in the short term
Long-term corrective measures
Long-term actions that need to be undertaken to prevent recurrence of the accident/incident
4 STATEMENT AND APPENDICES
If necessary copies of supporting evidence or statements (witnesses, injured party, experts etc) may be attached to the back of the report.HELPFUL TIPS
The summary of findings in the synopsis should be a condensed summary of the three contributing factors i.e. material/equipment factors, environmental factors, people factors.
Contributing factors are exactly that i.e. FACTS keep them brief and succinct.
Do not resort to the obvious conclusion regarding the cause of an accident. Assess all the facts.
Record accident details as quickly as possible. The longer the delay in gathering the accident details, the less accurate it is likely to be.
Consult with appropriate staff and if possible the injured party on suggested short and long term control measures.
The immediate supervisor (eg teacher, registrar, principal or manager) concerned is responsible for initiating an accident investigation and report. This responsibility may be shared with the school or workplace health and safety committee.
