白血病患者健康相关生命质量状况的调查分析 摘要：目的 了解上海市急性白血病 ( AL ) 长期生存病人的生命质量。方法 上海市1984.1.1 ～ 1994.12.31期间29 家医院1379例上海市病人中选择生存时间≥ 5年的95例长期生存患者为研究对象。用统一的FACT 2 G量表了解长期生存病人的生命质量。结果和结论 发现大多数病人基本达到正常的生命质量。其总的生命质量评分为85分。年龄大于60岁和文盲病人的生命质量评分比较高。近 50% 病人恢复正常的全日工作和学习。12.6% 病人有化疗后继发疾病，已婚病人中近一半病人对性生活不满意，所以预防继发疾病和保护性功能应引起足够的重视。复发和治疗经费是长期生存病人最担心的问题，建立医疗保险制度已成为社会问题。 关键词：急性白血病；长期生存；生命质量 Abstract: Objective To assess the quality of life ( QOL ) in long 2 term survivors w ith AL. M ethods There were 95 long 2 term survivors w ith AL drawn from 29 hospitals in Shanghai during 1984 to 1994. Questionnaire FACT 2 Gwas completed in order to as 2 sess the QOL for long 2 term survivorsw ith AL. Resultsand Conclusion Questionnaireswere filled in by 95 long 2 term survivorswith AL. The majority of survivors had normal QOL. The average QOL score was 85. Patients， being illiterate and aged over 60 years old， had higher QOL score. About 50% patients returned to full 2 time job or school. 12. 6% patients had secondary diseases due to chemotherapy. In married patients， nearly half of the patients were not satisfied w ith sexual life. Therefore， the prevention of se 2 condary diseases and the protection of sexual function must be taken seriously during chemotherapy. The most worrying problem s were relapse and treatment expenses. So， the establishment of medicine insurance system was very important for these patients. Key words: Acute leukem ia; Long 2 term survival; Quality of life 目前生命质量 ( QOL ) 在急性、慢性白血病中主要用于了解白血病治疗效果及不良反应、选择治疗方案、了解影响疗效的因素等。国内对血液系统肿瘤。如白血病、淋巴瘤等的研究较少。很少有自己的量表，大部分借用国外的量表。本研究目的在于制定适合中国国情的量表，初步评价信度和效度。并用此量表评价上海市AL 5年长期生存者的QOL情况。 1 材料和方法 1.1 研究对象　上海市白血病协作组1984～1994年期间达到长期生存5年以上的上海市区AL病人95例作为生活质量调查的研究对象，所有病人至少停用化疗半年。95例病人中AML 53例(55.8% )，ALL 42 例(44.2% )， 其中 M1 1例，M 2 10例，M 3 27例，M 4 6例 ，M 5 7例，M 6 2例，L1 20 例， L， L3 2(10～71 岁),男性 48 例，女性 47 例。 45 例(47.4% )恢复原来全日工作或上学，其中儿童、青少年全部恢复上学，8 例(8.4% )为半日工作，27 例(28.4% )病休 ,15 例(15.8% )退休在家。有 2 例病人因患白血病而离婚。 2.4 不同年龄和性别病人 QOL 评分的比较95例病人总 QOL 评分为 85 分，儿童(0～ 14
