案例讨论 四、移动脚手架,摔死 1、事故经过: 2001年11月3目,上海某清洗公司与川沙某集团公司签订厂房杠框架清洗油漆业务的合同和安全协议,11月球5日清洗公司组织了8名施工人员,由卜某作为现场施工负责人兼安全员。11月18日上午,施工人员冯某与周某、徐某三人一组,继续对屋顶框架进行油漆作业。中午11时左右,甲方通知午餐并发放了午餐票,在移动脚手架上作业的冯某和周某提出做完剩下的活再吃饭,两人没有下来就叫地面上的辅助人员徐某抒脚手架再往前移动。徐某松开固定装置,用力 移动脚手架。没有注意到上方针政4米处有两根管道横着。脚手架在移动中碰到管道受阻倾倒,使在脚手架上的冯周二人随之坠落在地。急送医院抢救,周某肩骨骨折,而冯某因后脑着地伤势严重,经抢救无效死亡。 案例讨论 四、移动脚手架,摔死 2、事故原因 按规定移动脚手架时上面的作业人员必须先下来,但实际操作时,施工人员图方便,人没有下来就移动脚手架, 造成事故的主要原因。 在移动脚手架时,施工人员未对周围环境仔细观察,忽宙了上方两根管道将阻碍正常移动的情况。 THE END KEY POINTS : From a management perspective, safety can be either reactive or pro-active. Many reactive companies believe that accidents are simply a cost of doing business and wait around for accidents to happen. Only when the volume and costs of accidents get out of hand are they motivated to focus on safety as a manageable part of the business. On the other hand there are ‘pro-active’ companies that firmly believe that accidents and incidents are preventable and demand that reasonable steps be taken to ensure worker safety. We need to adopt a firm belief that with sufficient forethought and planning, competent employees, proper equipment and materials to work with, we can succeed at preventing unnecessary losses such as injuries and equipment damage. History continually reminds us that a vast majority of accidents and incidents were indeed preventable and did not need to happen. We are intent on being pro-active at CACT.
