慢性阻塞性肺部疾病家庭氧疗依从性 的影响因素及护理对策 摘 要 慢性阻塞性肺部疾病(COPD)是严重危害人民群众健康的常见慢性病，其病程长，病理改变属于不可逆的损害，常并发肺心病和呼吸衰竭，影响患者生活质量甚至威胁生命，病死率在10%-15%。抗感染、舒张支气管、祛痰、吸氧等治疗是改善症状、控制病情发展的重要措施，而长期家庭氧疗（LTOT）是康复的主要措施之一。但我国家庭氧疗开展比较晚，目前依从性比较差。本文探讨了COPD患者长期家庭氧疗（LTOT）依从性主要跟患者的知识缺乏、文化程度、疾病的严重程度、治疗中的不适感、经济条件、家庭系统的支持以及医护人员等因素有关。为提高其依丛性应医护人员应加强对COPD患者及家属进行LTOT相关知识的宣教，尽量减轻患者氧疗中的不适感，给予正确的用氧指导，同时给予患者社会，家庭系统的支持，从而使LTOT达到真正的治疗意义。 关键词 慢性阻塞性肺部疾病；家庭氧疗；依从性；影响因素；对策 ABSTRACT ? Chronic obstructive pulmonary disease (COPD) is?a common chronic disease which is?serious dangerous to the health of?people, of which course is long and pathological change is irreversible damage. It is often complicated by pulmonary heart disease and respiratory failure,?which impacts the life quality of patient and even threats the life, whose mortality is10%-15%. Anti-infection, bronchial diastolic, expectorant,?oxygen therapy?and so on?are important measures which can improve symptom and control the development of disease, while long-term home oxygen therapy(LTOT) is one of the major significant rehabilitation. But home oxygen therapy is carried?out relatively lately in my country. Now the compliance is relatively poor. This article discusses the relationship among compliance of patients with COPD on LTOT and some factors which include the lack of?knowledge of patients, education of patients, the severity of the disease, discomfort during treatment, economic conditions, support of family system and nurses, and so on. In order to improve its compliance, nurses should strengthen to give relative LTOT knowledge to patients with COPD and their families, minimize the discomfort of patients in oxygen therapy, give the correct guidance on using oxygen and provide patients with social and family systematic support so that LTOT? can reach the real significance of treatment.
