胃癌术后复发相关因素探析[摘要] 目的：探讨影响胃癌术后复发的相关因素。方法：回顾性分析我院1997～2008年收治的胃癌术后复发病例的临床资料，对可能影响胃癌术后复发的相关因素进行分析。结果：复发时间5年者3例（7.9%），约有65.8%于术后2年内复发。T1 3例（7.9%），T216例（42.1%）， T3 19例（50.0%）。N0 2例（5.3%）， N1 19例（50.0%） N217例（44.7%）。Ⅰ期4例（10.5%），Ⅱ期9例（23.7%），Ⅲ期25例（65.8%）。结论：影响胃癌术后复发因素众多，浆膜侵犯、淋巴结转移是重要的因素。 [关键词] 胃癌；术后复发；相关因素 [中图分类号]R735.2 [文献标识码] B[文章编号] 1673-7210（2009）03（b）-138-02 Discussion about correlation factors of cancer recurrence after the stomach cancer surgery QUAN Bin, ZHANG Bo, YUAN Jianbao, KANG Jian (Department of General Surgery,the People’s Hospital of Shimen County,Hunan Province,Shimen 415300,China) [Abstract] Objective: To discuss the correlation factors of cancer recurrence after stomach cancer surgery. Methods: The data of patients with cancer recurrence after stomach cancer surgery from 1997 to 2008 was analyzed, then to study the factors related to recurrence. Results: Recurrence time 5 years,3 casses(7.9%).About 65.8% cases recurred no more than 2 years after operation.T1 3 cases(7.9%), T2 16 cases(42.1%),T3 19 cases(50.0%).N0 2 cases(5.3%),N1 19 cases (50.0%) N2 17 cases(44.7%).StageⅠ,4 cases(10.5%),stage Ⅱ,9 cases(23.7%),stage Ⅲ 25 cases(65.8%). Conclusion: There was many factors related to the recurrence after stomach cancer operation,among them,serosa infringement,the lymph node shift are the important factors. [Key words] Stomach cancer; Recurrence after operation; Correlation factors 手术是治疗胃癌的主要方法之一，但术后复发仍是影响预后的重要原因，有文献显示胃癌术后肿瘤复发与多种因素有关。本文对我院1997～2008年收治的胃癌根治术后复发病例的临床资料进行回顾性分析，探讨影响胃癌术后复发的相关因素，以期改进治疗方法，改善治疗效果。 1 资料与方法 1.1一般资料 本组患者38例，其中,男28例，女10例；年龄26～72岁，平均54.5岁。远端胃根治性切除术31例（81.6%），全胃根治性切除术后7例（18.4%）；其中，D1淋巴结清扫13例（34.2%），D2淋巴结清扫25例（65.8%）。术后全程接受规律全身静脉化疗25例（65.8%）。确诊方法主要是电子胃镜加取组织活检及腹部CT。 1.2 临床病理资料 依照国际抗癌联盟（UICC）1997版分期标准进行分期，其中T分级：T1 3例（7.9%），T2 16例（42.1%）， T3 19例（50.0%）；N分级：N0 2例（5.3%）， N1 19例（50.0%） N2 17例（44.7%）；M分级：M0 38例，M1 0例。Ⅰ期4例（10.5%），Ⅱ期9例（23.7%），Ⅲ期25例（65.8%）。其中，高分化腺癌 6例（15.8%），中分化腺癌14例（36.9%），低分化腺癌17例（44.7%），印戒细胞癌1例
