口腔内科学1061(Oral medicine, 1061) I. overview (1) the etiology of dental pulp disease There are many causes of dental pulp disease, including infection factors, physical and chemical factors, among which the most important pathogenic factor is bacterial infection. 1. bacterial factors (1) bacterial species: inflammation, bacteria in pulp are not specific. The infection of dental pulp comes from dentin deep caries, where the relative anoxic environment is favorable for the growth and reproduction of anaerobic bacteria. Therefore, the bacteria isolated from the pulp of inflammation are mainly facultative anaerobic cocci and obligate anaerobes, such as Streptococcus, actinomyces, Lactobacillus, and G - bacillus. The deep dentin layer is a rather anoxic environment which facilitates the growth and reproduction of facultative and obligate anaerobes. In the inflamed pulp cavity, many bacteria in the mouth, including fungi, can be detected in the inflamed dental pulp. But anaerobic bacteria are rarely detectable. In general, the degree of inflammation of the pulp is positively correlated with the number and duration of bacterial infection. (2) the route of infection: there are three ways for bacteria to invade dental pulp: 1) infection through the teeth is the most common route of infection. Dentin tubules: when caries involves deep dentin, the toxic products of bacteria can cause inflammation of the dental pulp through the dentinal tubules until the dental pulp is infected. Research has shown that when bacteria in the dentin from the pulp is less than 1.1mm (including reparative dentin, dental pulp) can have mild inflammation; when the bacteria from the pulp pulp was less than 0.5mm, may have the obvious inflammation; in the remaining dentin thickness less than 0.2mm, can be found in the pulp of bacteria.
