胰胃吻合术和胰肠吻合术应用于胰十二指肠切除术效果比较 　　[摘要] 目的 评价胰胃吻合术与胰肠吻合术应用于胰十二指肠切除术中的临床效果。 方法 选取2010年4月～2013年3月本院胃肠胰腺外科施行胰十二指肠切除术31例手术患者作为研究对象，根据术中胰腺残端的不同处理方式分为胰肠吻合术和胰胃吻合术两组，其中胰肠吻合术14例，胰胃吻合术17例。对两组患者手术时间、出血量、输血量、胰腺断面质地和术后并发症，包括胰瘘、胆瘘、腹腔出血、呼吸道感染、泌尿道感染、术后死亡的发生率进行比较分析。 结果 两组患者手术时间、术中出血量、输血量、胰腺断面质地、胰管直径的比较差异无统计学意义（P0.05），两组患者术后并发症（胰瘘、胆瘘、腹腔出血、消化道出血、呼吸道感染、泌尿道感染、术后死亡）的比较差异无统计学意义（P0.05）。 结论 采用胰胃吻合术后并发症发生率未高于胰肠吻合术，其可以作为胰十二指肠术后消化道重建的一种方法。 [关键词] 胰十二指肠切除术；胰胃吻合术；胰肠吻合术 [中图分类号] R656.6+1 [文献标识码] A [文章编号] 1674-4721（2013）11（c）-0037-02 Effect comparison of pancreaticogastrostomy and pancreaticojejunostomy applied to pancreatoduodenectomy YING Yong LUO Jia-fang ZHANG Wen-hong WANG Xiao-nong HE Xiao The First Affiliated Hospital of Gannan Medical College in Jiangxi Province，Ganzhou 341000， China [Abstract] Objective To evaluate clinical effect of pancreaticogastrostomy and pancreaticojejunostomy applied to pancreatoduodenectomy.Methods 31 cases from department of gastrointestinal pancreatic surgery in our hospital from April 2010 to March 2013 were collected as the research object and divided into two groups according to the different treatment to pancreatic remnant，14 patients underwent operation panereaticojejunostomy and 17 patients underwent pancreaticogastrostomy.Operative time，blood loss volume，blood transfusion amount，texture of pancreatic section and postoperative complications of two groups were compared and analysed，postoperative complications including pancreatic fistula，biliary fistula，abdominal cavity hemorrhage，respiratory infections，urinary tract infection，postoperative death.Results The operative time，blood loss volume，blood transfusion rate，the texture of pancreatic section of two groups had no statistical difference（P0.05），postoperative complications including pancreatic fistula，biliary fistula，abdominal cavity hemorrhage，respiratory infections，urinary tract infection，incidence of postoperative death of two groups had no statistical difference（P0.05）.Conclusion The incidenc
