Pre-Surrender Compliance Report INFORMATION FOR COMPLETING THIS FORM Purpose of this form A Pre-Surrender Compliance Report must be completed by an accredited exporter making an application to Wheat Exports Australia (WEA) under section 38 of the Wheat Export Accreditation Scheme 2008 (the Scheme) for consent to surrender its accreditation. Basis for this form Section 38(7) of the Scheme provides that a Pre-Surrender Compliance Report must be in a form approved by WEA. This form has been approved by WEA under section 44 of the Scheme. False or misleading information or documents WEA may decide to cancel or suspend an exporters accreditation under the Scheme if WEA is satisfied that the exporter provided false or misleading information to WEA. Giving false or misleading information is a serious offence. Providing false or misleading documents is a serious offence. Use of this form Exporters should provide the information requested in the spaces provided on this form. Responses can be handwritten or typed. Where boxes are provided please tick or check the appropriate boxes. If using Microsoft Word to complete the form, applicants can click on the box with the mouse to check the box. Attachment of documents All additional information or documents attached to this form must have a completed Attachment Cover Sheet (see Attachment A). Privacy WEA is collecting personal information as defined under the Privacy Act 1998 for the purpose of administering the Scheme. This includes for the purposes of making decisions, granting, suspending or canceling accreditation or making other decisions or taking other actions under or in relation to the administration of the Scheme. WEA usually gives some or all of the information collected to relevant Commonwealth, State or Territory entities or other entities for the purposes of making those decisions or taking those actions.
