Regular on-site service by the designated CPN. Services delivered: -mental state assessment and screening to clients -psychiatric nursing intervention -discussion with Homes’ staff on case management -skill transfer to Homes’ staff e.g. communication skill, management of behavioral problem Review on service provision with CPNs and Homes’ staff conducted using satisfaction survey Mental State Assessment Psychiatric Nursing Intervention Discussion on case management Framework of the service delivery model Result 1. Educational Activities: 14 briefing and educational talks conducted 96 staff attendance on management of elderly with mental problems 3.Satisfaction survey from Homes staff Key findings: Appreciated the close partnership with the designated CPNs Treasured the service in providing channel in seeking mental health advices and support.
