* * 1. Chronic Heart Failure patients will evolve over time towards more serious stages of Heart Failure. This evolution can occur more or less rapidly depending on the patient and his/her treatment. 2. During this evolution, there may be periods of stability but Heart Failure patients can also experience sudden changes in their symptoms which necessitate urgent hospitalisation. This is what we call Acute Heart Failure. 3. Forgetting to take medications, eating a salt rich dish, pulmonary infections or arrythmias are known to be precipitating factors for hospitalisation for HF. Almost half of
