Unit Ten CASE REPORT ——Retreatment Using a Modified-Tip Instrument Retreatments are common indications in the everyday life of an endodontic practice. The removal of old fillings, pastes, and obstructions may be attempted with the use of classical solvents such as chloroform and xylene (1,2). However, the removal of hard pastes represents a technical problem, because they are not soluble, they cannot be bypassed, and they block the root canal system. Ultrasonic instrumentation has been suggested in the literature (3-6) as effective for retreatments. It allows the removal of hard paste (3) without the need for surgical intervention. However, the process is time-consuming and requires an extra expense with the ultrasonic unit (3). A technique for removing hard pastes from the root canal is presented. The use of a modified-tip file has proven to be effective, easy, and fast. This technique allowed the retreatment of four difficult cases without surgical intervention, as shown herein. An orthodontic wire cutter is used to cut off the tip (4mm) of K-type files (# 30 and # 35). Because K-type files present quadrangular sections, trimming the tip off with the wire cutter produces a sharp edge at the working end of the file. This procedure turns the K-type file into a potent instrument to be used with apical pressure and reaming motion.
