Dep artment of Radiology, West C ina Hosp ital, Sic uan University, C engdu 610041, C ina Abstract Objective To investigate the spiral CT manifestations of the collateral circ lation pathw ays res lting from splenic vein occl sion ( SVO) d o to pancreatic diseases. Methods The CT imaging and clinical data of 33 cases of pancreatic disease with SVO, incl ding 28 cases of pancreat ic carcinoma, 3 cases of ac te pancreatitis and 2 cases of chronic pancreatitis, w ere retrospectively analyzed. Results Tort o s and dilated vessels were observed in the areas between splenic hil m and gastric f nd s and/ or along the gastric greater c rvat re in all 33 cases. In isolated SVO cases, the short gastric vein ( SGV, 86% ) , coronary vein ( CV, 79% ) , gastroepiploic vein ( GEV, 79% ) and gastrocolic tr nk ( GCT , 57% ) w ere varicose and dilated. While in non-isolated SVO, other collateral veins s ch as the right s perior colic vein ( RSCV, 37% ) , middle colic vein ( MCV, 37% ) and posterior s perior pancreaticod odenal vein ( PSPDV, 21% ) were seen as w ell. Conclusion T he two predominant collateral pathways of SVO are SGV gastric f ndal veins CV, and GEV GCT SMV. They have characteristic imaging feat res on spiral CT and are of clinical significance in both preoperative staging of pancreatic carcinoma and the
