1 375枚上消化道隆起病变内镜超声检查结果分析 　　作者：张明黎,吴正祥,张开光,丁西平,卫梅枝　　作者单位：安徽省卫生厅重点学科临床应用项目(2008B019) 230001 合肥 安徽省立医院消化内科 　　【摘要】目的 探讨内镜超声(EUS)对上消化道隆起病变的诊断价值，全面认识上消化道隆起病变。 方法 对1 375枚上消化道隆起病变进行内镜超声检查，分析超声图像特征及检查结果。 结果 1 375枚上消化道隆起病变都得到明确诊断，其中胃862枚，食管391枚，十二指肠122枚。依次为平滑肌瘤401枚、间质瘤128枚、壁外压迫342枚、囊肿79枚、血管瘤64枚、异位胰腺 58枚、脂肪瘤52枚、Brunner腺增生35枚、黏膜表面下腺瘤62枚、炎性增生51枚、黏膜表面下癌27枚、淋巴瘤7枚、静脉曲张7枚、十二指肠副乳头6枚、神经纤维瘤5枚、类癌2枚、胃黏膜肥厚症1枚、EUS未见异常48枚。除去壁外压迫，食管以平滑肌瘤、血管瘤为主;胃部以平滑肌瘤和间质瘤、囊肿、异位胰腺居多，十二指肠内Brunner腺增生最多。 结论 内镜超声检查对上消化道隆起病变有很高的定位诊断价值和较好的定性诊断价值，且可指导内镜下治疗，准确定性。 　　【关键词】 内镜超声; 隆起病变; 上消化道 　　The evalutoin of the endoscopic ultrasonography (EUS) to the 1375 cases with the apophysis lesions in upper digestive tract　　Zhang Mingli, Wu Zhengxiang, Zhang Kaiguang,et al 　　Department of Gastroenterology, Anhui Province Hospital, Hefei 230001 　　[Abstract] Objective To probe the diagnosis value endoscopic ultrasonography (EUS) to the apophysis lesions in upper digestive tract and to recognize this kind of diseases comprehensively. Methods 1375 patients with the apophysis lesions in upper digestive tract were examined by endoscopic ultrasonography (EUS) and analyzed the ultrasonography pictures and examine results. Resealts 1375 patients with the apophysis lesions in upper digestive tract are all identified, among them: 862 lesions were located in the stomach, 391 cases in the esophagus, 122 cases in the duodenum and they are leiomyoma 401, gastrointestinal stromal tumor 128, protuberance from outside organ342, cyst79, angioma 64, ectopic pancrease 58, lipoma52, Brunners gland hyperplasia 35,submucosal adenoma 62, inflammatory hyperplasia 51, submucosal cancer 27,lymphoma 7,varicosis 7, vice duodenal papilla 6, neurofibroma 5, carcinoid 2, M?n?trier 1,no abnormal 48。Apart from protuberance from outside organ, leiomyoma and angioma are mainly located in esophagus, leiomyoma and stromal tumor, ectopic pancrease are most in stomach and Brunner gland hyperplasia are mainly in duodenum.
