　 关节镜手术治疗运动员髌下脂肪垫撞击综合征 覃健 侯之启* 郑民庆 余存泰 王新亮 鞠洪斌 [摘要] 　目的　探讨使用关节镜诊治运动员髌下脂肪垫撞击综合征的疗效。　方法　2002年9月～2007年2月，对31例膝关节Hoffa氏病运动员患者进行关节镜下髌下脂肪垫部分切除术，部分患者同时施行外侧支持带松解术,术后按Insall 膝关节评定量表评估疗效。结果　所有患者获得7～34月随访（平均18±4.8月）,疗效评价:优17例( 54.8% ) , 良8例( 25. 8% ) , 可4例(12.9% ),差2例(6.5% ) ,关节镜技术治疗髌下脂肪卡压综合征治愈率达80.7%。获得优良效果的运动员患者在9.2±3.7周(7.1±14.5周)后恢复正常训练和比赛.　结论　Hoffa氏病是引起运动员膝前痛的常见病因，关节镜下行髌下脂肪垫部分切除术治疗Hoffa氏病具有创伤小、康复快、疗效可靠的优点。 关键词关节镜，髌下脂肪垫, 手术 The arthroscopic surgery for athletes withf Hoffa’s Disease QIN Jian, HOU Zhi-qi*, ZHEN Min - qing , et al. Abstract: 　Objective　To explore the efficiency of arthroscopic surgery for athletes with Hoffa’s disease. Methods　From September 2002 to February 2007 , the infrapatellar fat pad excision under arthroscopy were performed on 31 athelets with Hoffa’s disease, part of whom underwent lateral retinaculum lysis and the outcome were evaluated by Insall score system.　Result　 All the cases were followed-up 7～34 months（average，18±4.8 months）. The outcome were excellent in 17 cases( 54.8% ), good in 8 cases( 25. 8% ),fair in 4 cases(12.9% ) and poor in 2 cases(6.5% ). The general effect of treatment was 80.7%, and the patients who get good results resumed routine exercise and competition 9.2±3.7 weeks(7.1±14.5weeks) after surgery. Conclusion To atheletes, Hoffa’s disease is the common causes for anterior pain of knee.　The infrapatellar fat pad excision under arthroscope is an effective and minimally invasive way for Hoffa’s disease.
