服务性动物运输申请书 Application for Service animal Transportation 编号000001 旅客姓名 Name 航班号/日期 Flt No. / Date 动物名称 Animal variety 航程 Route 联运航班/日期 Connecting Flt / Date 联运承运人同意证明 Permission of successive carrier s 持有的旅行文件名称及编号： Certificate No. 声 明 旅客为所携带的安全、健康和管理承担全部责任，并应全权负责所携带与同机其他旅客之间可能发生的交流或问题。 携带的旅客应遵守所有相关的政府要求、规定或限制，包括运输该所涉及国家或地区的准入许可、检疫证明等。 旅客未能遵守以上要求规定，因未照管好所携带的而导致中国东方航空产生了任何财、物损失或其他费用，旅客应予以赔偿。 关于不符合相关资质要求的，中国东方航空在符合一般安全标准实施运输的情形下，不为该的患病、受伤以及由于该患病和受伤而导致的死亡承担责任。 旅客所携带的被相关国家或地区政府拒绝过境或入境，中国东方航空不为此而导致的损失或费用承担无限责任。 ?STATEMENT Passenger assume full responsibility for the safety, well-being, and conduct of his or her Service Animal and is solely responsible for its interaction with other Passengers on board the aircraft. Passenger is similarly responsible for compliance with all governmental requirements, regulations, or restrictions, including entry permits and required health certificates of the country, state, or territory from and/or to which Service Animal is being transported. Any passenger who fails to control their animal in accordance with the above mentioned rules shall reimburse China Eastern for any loss, damage, or expense arising out Service Animals behavior. With respect to Animals that do not qualify as service animals, China Eastern will not be liable for illness or injury to an animal or death of an animal due to illness or injury to the extent the animal has been handled by China Eastern with ordinary safety standards.
