不同方式固定跟骨骨折时螺钉应力分布特点 黄海晶1，辛景义1 【摘要】 目的：通过有限元模型，分析跟骨骨折不同部位固定螺钉所承受的应力，对不同内固定方式进行生物力学探讨。方法：在Mimics10.1中重建符合Dicom 3.0标准的跟骨CT图像，结合有限元软件ANSYS10.0生成最终的三维有限元模型。建立SOLIDWORKS模型，设计生成1枚接骨板，6枚接骨板冠状面固定螺钉及2枚跟骨矢状面固定螺钉。自足跟及跟腱附着点处分别垂直向上对模型施加320N和160N载荷，观察不同部位固定螺钉的Vonmises应力分布。结果：6枚接骨板冠状面固定螺钉中，距下关节面上部的螺钉在骨皮质处承受的载荷最大， 其他螺钉所受应力依次递减，跟骨前下端近跟骰关节处承受的载荷最小。2枚跟骨矢状面固定螺钉中，跟骨内1/3处固定螺钉所承受的应力达跟骨外1/3处螺钉的2倍以上。接骨板螺钉与矢状面固定螺钉在距下关节的应力分布明显不同，接骨板螺钉所承受的应力达矢状面螺钉6倍以上，具有对距下关节上部更强的支撑作用。 结论：钢板冠状面螺钉固定跟骨较矢状面螺钉对距下关节面有更强的支持作用，矢状面螺钉固定时内侧螺钉承载的应力较大。 关键词：有限元分析；跟骨；固定螺钉；应力分布 The stress distribution of screws when using different ways to treat calcaneus fractures Huang hai-jing1, Xin jing-yi1 (1. Orthopedic department of TianJin Hospital ,TianJin 300211) 【Abstract】 Objective:analyze the stress of the screws in different parts of calcaneal fractures by finite element model. Then evaluate the biomechanical effects of different internal fixation methods on the calcaneus. Methods: By using mimics10.1, the CT images of the bone were reconstructed according to the dicom 3.0 standard, with the help of the finite element software ansys10.0, a final three-dimensional finite element model – Solidworks model was generated for each bone. The generated virtual model includes 1 plate, 6 platescrews at the coronal plane of the calcaneus, and 2 fixative screws at the sagittal plane of the calcaneus. The model was loaded with vertical stress from heel and the insertional point of Achilles tendon (320N and 160N), the vonmises stress distribution on the screws in different site was observed. Results: Among six plate screws, the greatest stress was observed in the screw fixed above the surface of the subtalar joint. The stress on the other screws was descending proportionally in regard to the site. The smallest stress was observed in the screw that fixed in the sub-anterior calcaneus adjacent to the calcaneal-cuboid joint.
