高频超声对甲状腺腺瘤的诊断价值 2011级影像本科 1124300051 郑璇 【摘要】目的:探讨高频超声下甲状腺腺瘤的声像图特征以及超声对腺瘤的诊断价值，从而提高高频超声对甲状腺腺瘤的诊断准确性。方法：回顾性分析经手术或细针抽吸活检病理证实的 例甲状腺腺瘤患者 【Abstract 】Objective:The diagnostic value of high frequency ultrasound and thyroid tumor ultrasound images of thyroid tumor characteristics。The purpose of this study is to improve the rate of high-frequency ultrasound diagnosis of thyroid tumor diseaseMeth-ods:Analysis of 117 cases of thyroid adenoma patients with sonographic features, by surgery or needle aspiration biopsy, and will sonographic diagnosis and pathological findings contrast.Results:In the 117 cases, There are 105 cases of ultrasound diagnosis Retrospective analysis is correct，Misdiagnosed 12 cases；The main features of ultrasound images of thyroid tumors : it is circular or oval hypoechoic mass，Its shape is regular, clear boundary , internal echo uniform , surrounded by a hypoechoic ring surrounding。Surrounded by clumps of blood flow , and low resistance spectrum。onclusion:High frequency ultrasound can provide valuable information for the diagnosis and differential diagnosis of thyroid tumors , which improves the ultrasound doctors confidence. 【Key words Thyroid】: High frequency ultrasound; thyroid adenoma; diagnostic value 在甲状腺的所有疾病中，甲状腺腺瘤是较为普遍的疾病，收集了武警医院3年的关于甲状腺超声的检查，共有3088例，其中超声检查诊断结果为甲状腺结节的有1556例，诊断结果为甲状腺腺瘤的有443例，甲状腺恶性肿瘤有84例（很多病例不只一个诊断结果），其他的有1130例；所以尽早发现和区病灶的分良恶性对临床的治疗及手术选择有重要临床价值。而随着科学发展，我们对甲状腺腺瘤的检查方法也日益增多，例如：实验室检查、CT、MRI、高频超声、超声弹性成像、超声造影、穿刺活检等等，其中高频超声显像在诊断方面的应用尤其广泛[1]，因为它得到的信息更多价格也合理。 1.临床材料与方法 1.1 一般资料 收集乐山武警医院2015年1月份到2016年1月一年内的，经病理证实的117例甲状腺腺瘤的超声声像图资料及相应的病理结果和其他临床资料。在所入收集的病例中，男性有26例，女性有91例，年龄在11—78岁之间，平均年龄约为38岁。大多数患者是因无意中发现颈部无痛性包块或体检发现来医院就诊居多。 1.2仪器与检查方法 应用 GE S6/8、Philips IU22 彩色多普勒诊断仪，选取（7.5~12）MHz的高频探头，若肥胖患，可采用5MHz探头检查，医用超声耦合剂。 检查前患者不需特别准备，常规患者采取仰卧位，充分暴露颈部，使用足够的耦合剂使探头与皮肤密切接触，首先将探头沿甲状腺斜切、横切、上下切等三个方向无缝隙的进行探查，观察并测量甲状腺位置、形态、大小、实质回声等情况，详细的记录下病变的部位、数目、大小、形态、边缘、实质回声以及有无包膜、晕环等情况，还有是否有颈部淋巴结肿大等。然后加上彩色多普勒后观察腺体或者瘤体内的血流情况，测定最大流速以及阻力指数。必要时还可以结合实验室检查测定患者血清中T3、
