METHODS:nbsp; From 1990 to 2002, 214 patients with endometrial carcinoma in clinical stage I were treated with staging operation (SO group, n=134) and total hysterectomy plus bilateral sa1pingoophorectomy (HBSO group, n=80) respectively in our hospita1. Their clinicopathological data were ana1yzed retrospective1y. RESULTS: ①The 5year survival rate was 86.1% in HBSO group and 84.7% in SO group. No significant difference was found between the two groups (P＞0.05). Compared with the SO group, HBSO group was of significantly less complications (Plt;0.05). ②Among the l2 recurrence cases, there were 7 cases of distant metastasis and 5 cases of local recurrence. The recurrence and metastasis were related with the operation type. The local recurrence rate in HBSO group was significantly higher than that of SO group (Plt;0.01). ③The 5year survival rate of the cases with lymph node metastasis was significantly lower (42.3%) than that of ones without metastasis (75.5%) (Plt;0.01). The lymph node metastasis rate was related to cervical involvement, myometrial invasion ＞1/2, special pathologic type and histological low differentiation (G3) significantly. CONCLUSION: Lymphadenectomy for clinical stage I endometrial carcinoma results in more operation complications, but cant increase survival rate significantly. Lymphadenectomy is suitable for clinical stage I cases with high risk factors. Radiotherapy and chemotherapy are necessary for cases with lymph node metastasis. 　　【Keywords】 endometrial neoplasms; operation; treatment outcome; prognosis 　　【摘要】 目的: 探讨分期手术治疗子宫内膜癌的临床价值. 方法: 回顾性分析1990/2002分期手术(SO组,n=134)和全子宫加双附件切除术(HBSO组,n=80)治疗的214例临床Ⅰ期子宫内膜癌的临床病理资料. 结果:nbsp; ①临床Ⅰ期的HBSO组
