Registration mark assigned to the vehicle under regulation 6, 9(1), 13 or 14 of the Road Traffic (Registration and Licensing of Vehicles) Regulations (Cap 374 sub. leg.) or if no registration mark yet assigned, make and chassis number. 　　2. Name of policy holder. 　　3. Effective date of the commencement of Insurance for the purposes of the above Ordinance. 　　4. Date of expiry of Insurance. 　　5. Persons or classes of persons entitled to drive.* 　　6.
