[Method] From February 1995 ~ June 2007 there were 86 cases of 95 different causes of the complexity of hip hip lesions in total hip arthroplasty, using adequate preoperative preparation, assessment, simulation, surgery according to the specific circumstances in order to restore physiological and normal hip anatomy principle and conduct a total hip replacement. [Results] after 81 cases were followed up for 6 months to 10 years, an average of 3.5 years, according to Harris hip function score criteria, preoperative score 35 to 50 minutes, an average of 42.1 points, postoperative score 70 to 90 minutes an average of 82.3 points. Preoperative lower extremity severe unequal length in 2 cases, 1 case of 6 cm, the other 1 was 12 cm, after lower limb length inequality were 2 cm, 5 cm. Patients in this group 81 cases of lower limb length inequality for at least 1 cm, a maximum of 12 cm, an average of 4 cm, after cases of lower limb length discrepancy averaged 1.5 cm. 2 cases of poor wound healing surgical incision, splitting the upper femur in 3 cases, 1 case of prosthetic dislocation, sciatic nerve injury in 2 cases of deep vein thrombosis in 2 cases. [Conclusion] in complex total hip arthroplasty because of local variation in anatomical structure, reconstruction and rehabilitation of the hip with its focus on normal physiological structure and difficulties of adequate preoperative evaluation, simulation, operation in accordance with specific circumstances, the maximum restoration of hip anatomy and physiology, can obtain better clinical results.
