Methods of October 1999 ~ May 2007 in our hospital with pelvic fractures treated 42 cases of urethral injury , early rapid fluid resuscitation and transfusion to correct a large number of hemodynamic instability, unstable patients treated, underwent bilateral internal iliac artery ligation trunk and main branches of the dual control of pelvic fracture and urethral injury caused by bleeding. If the condition permits, emergency urethral anastomosis and bladder or urethral realignment traction gastrostomy fixation, if the critical condition, only with simple bladder fistulization. Results 27 cases of pelvic fracture urethral injury in patients with bleeding and shock, 26 cases of early under control, 1 died of uncontrolled bleeding .1 died late deep infection in .40 cases difficult to control after 1 to 9 years (mean 3.5 years follow-up, 24 patients cured of early urethral injury, complicated by urethral stricture in 12 cases, 4 cases of urinary incontinence and 6 cases of impotence. Conclusions and internal iliac artery ligation is a major branch of the dual control of bleeding pelvic fracture and urethral injury and effective measures. Early urethral anastomosis and bladder fistulization low incidence of urethral stricture, but the impotence higher rate of urethral realignment after more traction and fixation of urethral stricture caused by scar, but the narrow ends of dislocation from the urethra, reducing the difficulty of delayed operation, simply the inevitable legacy of bladder urethral stricture after colostomy, only for patients in critical condition .
