Methods:H.pyloriwasculturedfromendoscopicbiopsiesobtainedfrom281Tunisianpatients.ThevacAalleles, cagA,iceAandoipAgenotypesweredeterminedbyPCR. Results:ThevacAs1m1,s1m2ands2m2wererespectivelyfoundin10.7%,12.5%and45.6%ofstrains.Thes2m1 genotypewasnotdetectedinourstudy.ThecagAwasfoundin61.6%ofisolates.TheiceA1andtheiceA2 genotypeswererespectivelyisolatedin60.2%andin16%ofstrains.TheoipAgenotypewasdetectedin90.8%of strains.ConsideringthevacAandiceAgenotypes,thepresenceofmultipleH.pyloristrainsinasinglebiopsy specimenwasfoundrespectivelyin31.4%and23.8%.Thecomparisonbetweenstrainsisolatedfromantrumand fundusshowedthatTunisianpatientswereinfectedwithtwoormorestrainsofdifferentcagA,vacA,iceAand oipAgenotypesandthediscordancewasrespectivelyin9.6%,4.6%,8.9%and8.5%ofstrains. Conclusion:Ourresultsshowedthatin46%(131strainsamong281),theH.pyloristrainswerehighlyvirulentin relationofthethreeorfourvirulentfactorstheycouldcarry.Thesefindingweredescribedbeforeintheliterature. TunisianpatientswerecolonizedbyoneormultiplestrainsofH.pyloriinthesametimeinrelationofpresenceof vacAm1/m2andiceA1/iceA2inthesamebiopsy.Thediscordance
