Methods:Aprospective,multicenter,observationalstudywascarriedoutinsixSpanishICUs.Weincluded96 patientswithseveresepsis.WedeterminedthemtDNAhaplogroup,theCOXspecificactivity/citratesynthase specificactivity(COXa/CSa)ratioandtheCOXquantity/citratesynthasespecificactivity(COXq/CSa)ratioin circulatingplateletsatthetimeofdiagnosis,day4andday8.Weusedsurvivalat1and6monthsasendpoints. Results:PatientswiththeJTmtDNAhaplogroup(n=15)showedhigherCOXq/CSaratioatday4(P=0.04)and day8(P=0.02)thanthosewithotherhaplogroups(n=81).LogisticregressionanalysisshowedthattheJT mtDNAhaplogroup(oddsratio=0.18;95%confidenceinterval=0.04to0.94;P=0.04)andCOXq/CSaratio(odds ratio=0.53;95%confidenceinterval=0.30to0.93;P=0.03)wereassociatedwith1-monthsurvivalafter controllingforageandlacticacidlevels. Conclusions:Thenovelfindingsofourstudyarethat1-monthsurvivingsepticpatientsshowedhigherCOXq/CSa ratiothannonsurvivingindividuals,thatpatientsfromtheJTmtDNAhaplogroupshowedahigherCOXq/CSaratio andthatJTpatientshadahigher1-monthsurvivalthanpatientsfromothermtDNAhaplogroups.
