Methods:Twenty-fourtypeIdiabetesmellitus(DM1)withosteopenia(10femalesand14males)werecompared tothirty-eightage-andsex-matchedhealthycontrolindividuals(20femalesand18males)forbiochemicaland radiologicparametersofbonemass.Laboratoryinvestigationsincludedserumandurinarycalcium,inorganic phosphorus,alkalinephosphatase,andserum“procollagentype1N-terminalpropeptide(P1NP).Bone densitometrywasassessedatneckfemurusingDualEnergyX-rayAbsorptiometry(DEXA).SerumP1NPandDEXA werereevaluatedafteraplannedexerciseprogram. Results:Patientsandcontrolswerecomparablewithrespecttoserumaswellasurinarybiochemicalparameters ofbonemassnamely;calcium,phosphorusandtotalserumalkalinephosphatase.OsteopenicDM1patients displayedlowermeanserumP1NPthancontrolgroup(20.11±6.72ug\dLversus64.96±34.89ug\dL;p0.05).A significantcorrelationwasobservedbetweenBMDanddegreeofglycemiccontrolreflectedbyserumglycated hemoglobin(r=-0.44,p,0.030).BonedensitometrycorrelatedwithserumP1NP(r=-0.508,p,0.011).Aftera plannedregularexercisefor3months,serumP1NPandBMDlevelsincreasedwithpercentagechangeof40.88± 31.73and3.36±2.94,respectively.Fivepatientsresumednormaldensitometryandtheywereallmales. Conclusion:Diabeticosteopenicpatientsdisplayedlowerserumlevelsofprocollagentype1N-terminal propeptidewhichreflectspoorboneformation.A3-monthsplannedexerciseprogramwasassociatedwith improvementofbonedensitometryandsignificantincrementofserumP1NP.
