Abstract
Background:Feverisoneofthemostcommonpresentingcomplaintsinpaediatricsandgeneralpractice.Inthe
majorityofcasesnothingharmfulisdiagnosed.However,thesubjectivemeaningoffeveroftenvariesbetween
doctorsandparents.Knowledgeoftheparents’conceptoffevermayhelptailorcounsellingtotheirneeds.
Inthisstudywedetermine1)theinfluenceofsocio-economicstatusandculturalbackgroundontwoconceptsof
feverwhichwelabelled“functional”and“fearful”,eachrepresentingtypicalexperiencesofmothers,and2)the
actionstakenbythemothersrelatedtotheseconcepts.
Methods:AstandardizedinterviewstudywasconductedamongGermanandTurkishmothersinGermanyin
2009.Thequestionnaireconsistedof36questionsand205items.Interviewswereconductedin16private
practicesofpaediatriciansand2paediatricemergencydepartmentsinanurbanregionofGermany.Thetwofever
conceptswererepresentedin6statementsthatcouldberatedwithasix-pointLikertscale.Theassociationofthe
socio-economicstatusandtheculturalbackgroundwithoneofthefeverconceptswasdeterminedbyamultiple
logisticregression.
Results:Atotalof338mothers(49%withaTurkishbackground)completedtheinterview(responserate92%).The
averageageofmotherswithaGermanbackgroundwashigher(34.1yearsvs.32.0years,p=0.0001).Motherswith
aTurkishbackgroundweremorelikelytorelatetotheconcept“fearful”[adjustedOddsRatio(AOR)1.99;
confidenceinterval(CI)1.16-3.44].Motherswithamiddleorhighsocio-economicstatusweremorelikelyto
respondtotheconcept“functional”[middle:AOR,0.53;CI,0.30-0.92;high:AOR,0.44;CI,0.21-0.95].
Mothersadheringtotheconcept“fearful”moreoftengaveacetaminophenbeforetherecommendedintervalof6
hours(46.8%vs.31.3%,p=0.005)andvisitedout-of-hoursservicesmorefrequentlyinthepreceding9months
thantheothergroup(0.7vs.0.4,p=0.001).
