消化内镜消毒效果的影响因素及对策 【摘要】 为防止经内镜传播疾病，我们对消化内镜从清洗消毒到诊疗全过程进行了分析，发现清洗消毒过程中操作不规范、消毒液浓度不足、消毒过程中一些细节未注意、内镜保存不当、病人未分诊等是影响消化内镜消毒效果的主要因素。针对其影响因素采取了有效对策，如：加强内镜的消毒管理，杜绝操作不规范的一切因素，加强消毒液的监测，注意酶洗液的应用，注意消毒过程中的细节问题及消毒后内镜的保存，加强诊疗病人的分诊。 【关键词】 消化内镜；消毒效果；质量；院内感染 ［Abstract］ To prevent endoscopic transmitted diseases,the cleaning and disinfection of endoscopic treatment to the entire process was analyzed and we found that the process of cleaning and disinfection operation was not standardized,disinfectant concentration was inadequate,in disinfection process we did not pay attention to some details,endoscopic preservation was improper,triage was not at the patient affecting digestive endoscope disinfection efficacy were the main factors.For thEir influence factors we had taken effective measures such as:strengthening the disinfection of endoscopic management and eliminating all non-standard operations,strengthening the monitoring of disinfection,paying an attention to application of lotion,paying attention to the procoss of sterilization and disinfection of the details of the endoscope after the preservation and strengthening the patients,triage,we conducted on a regular basis endoscopic biological monitoring to ensure the quality of endoscopic disinfection to prevent hospital infection and the occurrence of the spread of the disease. ［Key words］ gastroendoscope;disinfection;quality;nosocomial infections 随着医学的 发展 进步，消化内镜在临床诊断和 治疗 上得到了较广的应用，但通过内镜传播疾病亦时有发生，感染率在不断增加，成为导致 医院 感染的一个新的不容忽视的问题，因而内镜的清洗消毒问题已越来越引起医学界和患者关注［1］。而影响内镜消毒效果的因素很多，有些是工作中易勿视的、有些是人为的因素，要清楚地了解，才能防患于未然，我们通过对消化内镜从清洗消毒到诊疗的全过程进行分析， 总结 出了其影响因素及对策，现介绍如下。 1 消化内镜消毒效果的影响因素 1.1 操作不规范，未达到彻底清洗消毒的目的 （1）内镜室护士未经专业培训，致操作不规范、清洗不彻底，残留有机物［2］而影响消毒效果；（2）护士素质不高，缺乏必要的医德修养，不按操作规范执行清洗消毒。（3）存在供需矛盾：需要做胃镜检查的病人多，而镜子少，造成清洗不彻底、消毒时间不足。 1.2 消毒液浓度不够，致消毒效果受影响 （1）多数医院使用的2%碱性戊二醛，可因保存不当、有效期已过、反复浸泡消毒次数过多等而影响消毒效果。（2）近年逐渐被许多医院采用的一种绿色环保型消毒剂酸性氧化电位水（EOW），起杀菌作用的是ORP值、pH值、有效氯含量，而其影响因素是食盐浓度、电解时间及储存条件，而储存时的温度、时间、密闭情况、振荡等都可影响消毒液的杀菌效果［3］。 1.3 多酶液浓度不够或未用 以致内镜腔道中的污物不
