结直肠癌36例 18 F-FDG符合显像结果分析 作者：袁建伟 冯彦林 余丰文 刘德军 贺小红 黄克敏 【关键词】 结直肠癌 【摘要】 目的 探讨 18 F-FDG符合显像在结直肠癌患者中的临床意义。 方法 回顾性 分析 36例（男20例，女16例）结直肠癌患者的 18 F-FDG符合显像结果。结果采用目测法并与局部CT/MRI、骨扫描及血清CEA等相比较。结果 36例病例中局部复发3例，术后瘢痕3例，远处转移27例。CEA阴性的结直肠癌病例中有7例复发和发生远处转移。 18 F-FDG符合显像发现了常规检查阴性的20个病灶并因此改变了9例（25%）依据传统检查方法制定的 治疗 方案。结论 18 F-FDG符合显像在诊断结直肠癌局部复发及血清CEA阴性的复发病例中有较高的诊断价值;并有利于临床的分期、治疗效果的随访及制定个体化的治疗方案。 关键词 结直肠癌 复发 符合显像 双探头 【Abstract】 Objective To evaluate the clinical value of 18 F-FDG coincidence imaging with dual-head（CIDH）camera in the diagnosis of coloretal cancer.Methods 36cases with colorectal cancer which imaged with CIDH were analyzed retrospectively. The results were compared to those of conventional examinations，which included serum CEA measurement，local CT/MRI scanning，radionuclide bone scintigraphy.Results Of36patients，local reˉcurrence was revealed in3，tissue scar in3and distant metastases27，respectively.Of the7patients with CEA negaˉtive 18 F-FDG imaging detected recurrence and distant metastases. 18 F-FDG imaging detected20unsuspected metasˉtases in conventional imaging modalities（CIM）and altered therapy in9（25%）of these36patients of the basis of the CIM results.Conclusion 18 F-FDG coincidence imaging is highly valuable in the diagnosis of local recurrence a CEA negative occult recurrence，and is also helpful in staging，monitoring response to treatment and selecting the apˉpropriate treatment protocols to colorectal cancer. Key words colorectal cancer recurrence coincidence imaging dual-head 结直肠癌是消化道常见的恶性肿瘤之一，近年来其发病率呈上升趋势且结直肠癌根治术后仍有30%～50%的病例发生复发和转移 ［1］ ，因此早期诊断及制定合理的治疗方案已成为临床上一个尤为重要的 问题 。与常规影像检查相比FDG符合显像能够提供活体组织的代谢信息，肿瘤组织的糖代谢增强的表现。许多 研究 证实，FDG符合显像在检测结直肠癌原发灶及其复发中具有重要价值 ［2］ 。本文回顾性分析了36例结直肠癌病例的FDG符合显像，以探讨FDG符合显像的临床意义。 1 资料与方法 1.1 一般资料 1.1.1 原发结直肠癌 12例 18 F-FDG符合显像阳性病例中最终经病理证实10例为原发结直肠癌，1例为结肠NHL，1例为回盲部溃疡。10例原发病例中，男7例，女3例。其中中分化腺癌3例，低分化腺癌4例，粘液腺癌2例，未分化癌1例。 1.1.2 结直肠癌术后 26例结直肠癌术后患者，男13例，女13例。其中3例局部复发，3例为术后瘢痕（经临床随访证实），远处转移20例。26例患者中CEA阴性的患者11例。 1.2 显像仪器和显像剂 采用美国ADAC公司Vertex plus V60MCD
