Abstract ObjectiveToevaluatetheeffectsofexenatideontheplasmaglucose,liverfatcontentandserum chemerininobesepatientswithtype2diabetesmellitus(T2DM)andnon-alcoholicfattyliver disease(NAFLD). Methods60casesofobesepatientswithT2DMandNAFLDwhowereunabletoachievebetter glycemiccontrolwithdietcontrol,exerciseandoralanti-diabetesagentsforatleast3monthwere randomlydividedintotwogroups.Onegroupwastreatedbyexenatide(10ugtwicedaily),theother groupwastreatedbyinsulinglargine(thestartingdosewas0.2U/kgor6-10U)for12weeks.The changeofbodyweight(BW),bodymassindex(BMI),fastingplasmaglucose （FPG）,glycosylated hemoglobin(HbA1c),2hoursplasmaglucose(2hPG),bloodlipid(TG、TC、HDL、LDL),insulin resistanceindex(HOMA-IR),liverenzymeindex(ALT,AST)andserumchemerinlevelwere measured;thechangeofliverfatcontent(LFC)werequantitatedbyultrasound. Result(1)10casesofexenatidegrouphavemildgastrointestinalreaction(includenausea,vomiting anddiarrhea), 1casefalloffbecauseofintolerance,1casewaslosttofollow-up;inthecontrolgroup twocaseswaslosttofollow-up,eventually56subjectsaccomplishedobservation.(2)Thelevelsofthe BW,BMIandWHRintheexenatidegroupweresignificantlydeclinedincomparisonwiththecontrol groupaftertreatment （P0.01）;theHbA1cdecreasedby 1.07%and1.02%,theFPGdecreasedby 1.49mmol/Land 1.06mmol/L,the2hPGdecreasedby4.16mmol/Land2.63mmol/L （P0.05） in exenatideandcontrolgroup;thelevelsofTG,ALT,ASTweredeclinedintheexenatidegroup （P0.05）;theFINSandHOMA-IRweredeclinedintheexenatidegroupcomparedwiththecontrol group （P0.01）;thelevelsofchemerin[(170.30±17.99)VS(142.67±17.45)]wasdeclinedinthe exenatidegroup,thecontrolgrouphasdescendingtrends[(163.53±16.80)VS(151.61±24.85)] （P0.05）;theLFCdecreasedby2.85% （P0.05）and1.38%(P0.05)intheexenatideandcontrol group.(3)Co
