Pit and fissure cavity Smooth surface cavity Buccal cervical lesions Occlusal caries advanced initial Occlusal Caries the high incidence of carries on these surfaces has been directly related to the narrow and inaccessible pits and fissure on occlusal surfaces, and for that reason it has been natural in the past simply to refer to occlusal caries as ‘fissure caries’. In the maxillary molar, for example, the central and the distal fossae are sites which typically accumulate plaque and hence also sites where caries most often occurs. In general terms occlusal caries initiation takes place in locations where bacterial accumulations receive the relatively best protection against functional wear. Thus two factors have been considered of importance for plaque accumulation and caries initiation on occlusal surface: (1) stage of eruption or functional usage of teeth, and (2) tooth-specific anatomy. Root surface caries comprises a continuum of changes ranging from minute, slightly softened and discolored areas on the root surface to extensive, yellow-brown very soft areas which can encircle the entire root surface. Even in rather large lesions, cavitation may be limited but, if untreated, root surface caries cavities can extend into the pulp chamber. Root surface caries Active root surface caries lesion. Any area which is well-defined and shows a yellowish or light-brown discoloration. The lesion is most likely covered by visible plaque and/or presents a softening or a leathery consistency on probing with moderate pressure. Inactive root surface lesion. Any root surface area which shows a well-defined dark-brownish or black discoloration. The surface of the lesion is smooth and shiny and appears hard on probing with moderate pressure. In both active and inactive lesions, cavity formation may be observed, but in the latter case the margins appear smooth. Rampant Caries Rampant is a rapid process involving a large number of teeth.
