临床研究 老年便秘患者350例肠镜结果分析( 吕 婵 ，岑 戎* (上海中医药大学附属曙光医院内镜室，上海 200021 ) 【摘 要】目的 探索老年便秘患者和内镜下发现的肠道疾病之间的关系。 方法 归纳上海中医药大学附属曙光医院2009年7月2013年7月350例老年便秘病例的结肠镜结果，结合老年便秘患者病程、性别归纳分析镜下肠道疾病的分布特点。 结果 350例老年便秘病例肠道疾病发现率高达60.86%，相关大肠疾病发病率依次为结直肠炎(26.86%)、结肠黑变病(13.14%)、肠息肉(12.00%)、结直肠癌(6.57%)、炎症性肠病(1.14%)、结肠憩室(0.57%)、肠结核(0.29%)、毛细血管扩张症(0.29%)。男性便秘病例肠息肉、结直肠癌检出率高于女性(P0.05)。便秘病程5年组结肠黑变病发生率较高(P0.05)。 结论 老年人便秘的最主要病因是结直肠疾病，尽早行结肠镜检查，明确病因，对症治疗，才是提高老年人生活质量的关键。 【关键词】便秘；结肠镜；老年R592；R574 【文献标识码】A 【DOI】 Analysis on colonoscopic results of elderly constipated patients: report of 350 cases LYU Chan, CEN Rong* (Department of Endoscopy, Shanghai Shuguang Hospital, Shanghai University of Traditional Chinese Medicine, Shanghai 200021, China) 【Abstract】 Objective To investigate the relationship between constipation and intestinal diseases diagnosed by endoscopy in aged patients. Methods The colonoscopic results of 350 aged patients (mean age of 72.29±15.71) with constipation in our department from July 2009 to July 2013 were collected and retrospectivley reviewed. The endoscopic features of intestinal diseases were analyzed with their disease course and gender. Results Among the 350 cases, the prevelance of intestinal diseases was 60.86% diagnosed with by endoscopy, among which were colorectitis (26.86%), melanosis coli (13.14%), colorectal polyps (12.00%), colorectal cancer (6.57%), inflammatory bowel disease (1.14%), colonic diverticula (0.57%), intestinal tuberculosis (0.29%) and colon vascular ectasias (0.29%) in order. The prevalences of colorectal polyps and cancer were higher in the male constipated patients than the female ones (P0.05). A higher incidence of melanosis coli was observed in the cases with constipation over 5 years (P0.05). Conclusions Constipation in the aged patients is mainly caused by colorectal diseases. Thus, colonoscopy for these patients is recommended to ensure early detection and treatment, which may be of great help in improving the aged patients’ quality
