中国厂矿医学2008年6月第21卷第3期 Chinese Medicine ofFactory and Mine，June 2008，Vo1．21，No．3 269 影响进展期胃癌根治性切除术患者预后的多因素分析 李志贵 彭汪送 周新华 (马鞍山十七冶医院普外科 安徽省马鞍山市 243000) 【摘要】 目的 探讨影响进展期胃癌根治性切除术患者的预后因素。方法 回顾性分析1996至2004年间收治的进展期胃癌 并实施根治性切除术的109例患者资料，用Kaplall—Meier法计算术后生存率，用Log—rank法进行单因素分析，以Cox模型进行多变量 分析。结果 患者术后1、3、5年累积生存率分别为99．1％、39．4％、10．1％。单因素分析显示：病理组织学类型、Borrmann分型、 pTNM分期影响术后累积生存率(PO．01)；癌肿部位、是否局部扩大淋巴结清扫、术前是否输血、是否联合脾脏切除不影响术后 累积生存率(P0．05)。多因素分析显示：病理组织学类型、Borrrnann分型、pTNM分期均为独立危险因素，危险性依次为pTNM 分期、Borrrnann分型、病理组织学类型。结论 病理组织学类型、Borrrnann分型、pTNM分期是影响进展期胃癌根治性切除术患者 预后的独立危险因素。 【关键词】 胃肿瘤；预后；多因素分析 中图分类号：R 735．2 文献标识码：A 文章编号：1004—7956(2008)03—0269一O3 Multivariate analysis of prognostic factors in pafien~ th advanced gastric carcinoma treated th the t0taI resection ，J Zhi— gui，PENG Wang·song，ZHOUXin—hua．Department ofGeneral Surgery，The／／osp／talof17thMetallurgyCorporation，Maanshan243000，China 【Abstract】 0bje~ive To analyze the prognostic factors in patients with advanced gastric carcinoma treated with the total resection． Methods The clinical data of109 cases with advanced gastric carcinoma treated with the total resection from 1996 to2004 year were analyzed retrospectively．The postoperative cumulative survival rates were counted by Kaplan—Meier method，univariate analysis of Log—rank method and multivariate Cox regression model were used to analyze the factors affecting the prognosis．Results The postoperative cumulative survival rates f0r 1 year，3 years and 5 years were 99．1％ ，39．4％ and 10．1％ respectively．Univariate analysis revealed that the histopathological type ，the Borrmann type and the pTNM stage affected the postoperative cumulative survival rate；but the tumour location，local extended clearation of lymph nodes，preoperative transfusion and joint resection of spleen didn’t affect the postoperative cumulative survival rate．Multivaria
