中华实用诊断与治疗杂志2008 年第22 卷第7 期 ·481 · ·论　著 · 外周血sICAM-1 联合AFP 检测对原发性肝癌诊断的意义 赵威,窦科峰,岳树强,杨雁灵 　　〔 要〕　目的:联合检测血清中甲胎蛋白和可溶性细胞间黏附因子-1, 以提高原发性肝癌的确诊率。方法:肝病住 院患者174 例中原发性肝癌46 例, 肝转移癌 18 例, 肝硬化42 例, 各型肝炎30 例, 良性肝肿瘤38 例以及健康对照者 46 例, 分别采用荧光酶联免疫吸附法和双抗体酶联免疫吸附法检测血清甲胎蛋白和可溶性细胞间黏附因子-1。 结果:甲胎 蛋白和可溶性细胞间黏附因子-1 联合检测使原发性肝癌的检出灵敏度达84%, 正确率达87%, 高于其中任一单项检测, 可明显降低漏诊率。结论:肿瘤标志物的联合检测对确诊原发性肝癌具有临床价值。 　　〔关键词〕　原发性肝癌;甲胎蛋白;可溶性细胞间黏附因子-1 　　中图分类号:R735.7 　　文献标识码:A 　　文章编号:1674-3474-(2008)07-481-03 Significance of detecting alpha-fetoprotein and soluble intercel lular adhesion molecule-1 in the diagnosis of primary hepatocellular carcinoma ZHAO Wei, DO U Kefeng, YUE huqiang, et al.Department of Hepatobillary Pancreas-spleen urgery, Xijing Hospital, the Fourth Military Medical University, Xi′an 710033, China 　　〔Abstract 〕Obj ective T o enhance the final diagnosis rate of primary hepatocellular carcinoma by detecting alpha-fetoprotein and soluble intercellular adhesion molecule-1.Methods One hundred and seventy-four patients w ith liver disease including 48 cases of primary hepatocellular carcinoma, 18 cases of liver metastatic carcinoma, 38 cases of liver benign tumor, 42 cases of hepatic cirrhosis, 30 cases of hepatitis and other 46 cases of normal controls were determined the sera alpha-fetoprotein and soluble intercellular adhesion molecule-1 w ith fluorescent enzyme linked immunosorbent assay and double antibody enzyme linked immunosorbent assay.Results Combined detection of alpha-fetoprotein and soluble intercellular adhesion molecule-1 enhanced the sensitivity of primary hepatocellular carcinoma diagnosis to 84 %and the accuracy to 87%, higher than those of detection of any single tumor marker, and obviously redu
