精品论文 参考文献 内镜下异物取出术中应用软性外套管的优势及其护理配合 曹艳　郎海波　张杰 通讯作者首都医科大学附属北京安贞医院消化内科　北京　１０００２９ 　　【摘要】　目的　探讨内镜下异物取出术中应用软性外套管的优势及其护理配合.方法　回顾性分析在２００５年１月至２０１３年１２月期间因怀疑有上消化道异物而在我院就诊的１２６例患者,比较应用不同内镜检查治疗方法后的并发症率、成功率和术中护理配合的方法.结果　对应用软性外套管治疗的３９例患者中,通过术中与医生密切配合,异物成功取出率为１００％,取出过程中出现食管轻度粘膜损伤的患者１例,为２．５６％.８７例患者接受传统内镜检查,治疗和护理方法,成功取出异物的患者６７例,成功率为７７．０１％,未能取出异物的患者２０例,为２２．９９％;将食管内异物推送入胃腔中为４例,发生嵌顿的有??３例,予外科手术的３例;传统内镜检查和治疗方法中食道上段粘膜发生出血及损伤的患者２０例,为２２．９９％.异物取出术使用内镜软性外套管成功率为１００％,并发症发生率为２．５６％;异物取出术未使用内镜软性外套管辅助的成功率为７７．０１％,并发症发生率为２２．９９％.结论:在应用软性外套管辅助治疗的异物取出术中,通过与医生的密切配合和精心细致的护理,能有效提高异物取出的成功率,是降低食管上段粘膜损伤,减少并发症产生的关键.该方法的使用能有效的减轻病人痛苦,简单,易行. 【关键词】　软性外套管;　消化道异物;　内镜;　护理Endoscopicforeignbodiesinapplicationtheadvantageofsoftoutercasingandnursingcooperation　YanCaoHaiBoLangjiezhang?　(Beijinganzhenhospitalaffiliatedtothecapitaluniversityof medicalsciences,digestiveinternalmedicine１０００２９)【Abstract】　Objective:tostudytheendoscopicforeignbodiesinapplicationtheadvantageofthesoftoutercasingandnursingcooperation．Methods:aretroＧspectiveanalysisinJanuary２００５toDecember２０１３inourhospitalbecauseofsuspectedofuppergastrointestinaltractforeignbodiesofthe１２６patints,comparetheapplicationofdifferentendoscopicexamination,successrateandcomplicationrateafterthetreatmentmethodsofintraoperativenursingcooperation．Results:thesoftoutercasingtreatmentof３９cases,throughintraoperativeinclosecoordinationwiththedoctor,foreignbodyremovedsuccessfullyatarateof１００％,apＧpearedintheprocessoftakeoutpatientswithesophagealmildmucosalinjuryin１case,２．５６％．８７patientsreceivingtraditionalendoscopicexamination,treatＧmentandnursingmethod,successfullyremoveforeignbodiesofthe６７casesofpatients,thesuccessratewas７７．０１％,２０patientswithfailedtoremoveforeignbody,２２．９９％;Esophagealforeignbodypushintothestomachcavityfor４cases,thereare１３casesofincarceratedin３casesofsurgery;Traditionalendoscopicexaminationandtreatmentofesophagealmucosahemorrhageoccursintheupper２０casesofpatients,anddamageof２２．９９％．Foreignbodiesusingendoscopicsoftoutsidecasingsuccessratewas１００％,theincidenceofcomplicat
